AFFIDAVIT

I, PAMELA JOAN PASCUA TIBURCIO, Filipino, of legal age, married and a resident of
Brgy. Cruz Victoria, Tarlac, after having been duly sworn to in accordance with law, do
hereby depose and say:

1. That I am the daughter of TERESITA DELFIN and SANTIAGO PASCUA,

who died on January 18, 2010 ;

2. That my parents owned several parcels of lot registered in their own name;

3. That I need to secure the death certificate and birth certificate of my father,
SANTIAGO PASCUA in Philippine Statistics Office;

4. That such requirements are needed for the filing of a Judicial Partition in court;

5. That I am executing this affidavit to attest to the truth of the foregoing statements
and inform the proper authority concerned about the said death and for whatever
legal purpose this affidavit may duly serve.

IN WITNESS whereof, I have hereunto set my hand this 17th day of August 2020 in
the Province of Tarlac.
