AFFIDAVIT

I, , of legal age, Filipino, single with residence and

postal address at 123 Orchid St. Brgy. Mabango, Makati City,
Metro Manila, after having duly sworn to in accordance with
law, do hereby depose and say:

That, I am presently employed at ABC Company, as

Accounting Officer;

That, I am designated at the accounting department

to assess financial decisions, implement accounting policies
and supervise accounting staff;

That the company’s inventory updates for the

month of January 2017 to April 2018, and May 2019, about
P1.2 million worth of ABC products were unaccounted;

That the staff of the Accounting Department would

frisk all warehouse personnel including W, X, Y and Z before
they would leave the premises of the company deliver the
perfumes to different stores;

That I am executing this affidavit to attest to the

truth of the foregoing facts and veracity of the foregoing
statements and for all legal intents and purposes it may serve.

IN WITNESS WHEREOF, I have hereunto set my
