Case II

You are admitting a client to the inpatient unit. The client is in a manic state exhibiting flight of
ideas, loose association, poor appetite, irritability, and rapid mood swings between elation and
crying spells. The client has been unable to sleep for the past three nights and stays awake
pacing the floor

1.Identify at least five questions you will ask during your initial interview.

 Tell me something about the events that took place to your admission to the hospital ?
 Do you feel like you can work all day without needing rest/sleep?
 Do your thoughts jump from place to place that makes it difficult for you to keep track of
them?
 During this period did you feel especially confident, like you were on top of the world?
 Did this feeling of unusually high energy and a decreased need for sleep go on not for
hours or an evening, but for days and days at a time?
 you ever felt that your mood was elevated or "too good" for a series of consecutive days?

2.List three main priorities of care for the client.

nerve/muscle cells; influences reuptake of serotonin/norepinephrine.

-Therapeutic Effect: Stabilizes mood, reducing episodes of mania.

4.If disturbed sleep pattern related to sensory alterations as evidenced by verbal complaints of
difficulty sleeping is one of your nursing diagnoses, identify one short-term outcome and three
nursing interventions for the nursing care plan.

Outcome:

1. after instruction by the nurse, the patient Verbalizes plan to implement bedtime
routines.

Interventions:

1. Encourage daytime physical activities but instruct the patient to avoid strenuous
activities before bedtime.
2. Prevent the patient from thinking about next day’s activities or any distracting thoughts at
bedtime.
3. discourage caffeine or large meal intake 2 hours before the patient goes to sleep.
4. Provide measures to take before bedtime to assist with sleep.
