AFFIDAVIT OF LOSS

I, ALYN GABUTAN LICAROS, Filipino, of legal age, single and with residence
at Banawa Riverside, Cebu City, Philippines, after being duly sworn to in accordance
with law, hereby depose and state:

1 That I am a duly licensed driver and I am driving a motorcycle;

2 That sometime in 2019, I owned a Lenovo Smartphone and I applied for a
Grab Food driver and consequently I had my GCash and other applications
installed in my smartphone;
3 However, such Lenovo Smartphone was lost and diligent efforts to locate the
said smartphone proved futile and it was beyond recovery;
4 That due to the loss, I was unable to start as a Grab Food driver;
5 That I am executing this affidavit to attest to the truth of the foregoing facts
and for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this January 27, 2021 at
Cebu City, Philippines.
