AFFIDAVIT TO USE SURNAME OF THE FATHER

I, MARK ALLEN MANABAT MAGPULONG, of legal age, Filipino, single

and a resident of MIV Phase III, Pulpogan, Consolacion, Cebu, Philippines, after
having been sworn to in accordance with law, do hereby depose and state:

1. That I am the father of MATTHEW ACE TRANGIA MAGPULONG;

2. That he was born on January 27, 2020 at Vicente Sotto Memorial

Hospital, B. Rodriguez Street, Cebu City, Philippines;

3. That the fact of his birth was duly recorded in the hospital where he was
born;

4. That being the father of MATTHEW ACE TRANGIA MAGPULONG, I

hereby acknowledge and recognize him as my child and legal heir;

5. That I hereby allow MATTHEW ACE TRANGIA MAGPULONG to use

my surname in accordance with R.A. 9255;

6. That I execute this affidavit to attest to the truth of the foregoing facts
and for whatever legal purpose this may serve.
