AFFIDAVIT OF CONSENT

I, ANALYN C. CORSIGA, Filipino, of Legal age, married and a resident of Brgy. Ulhay
Javier, Leyte, Philippines, having been duly sworn in accordance with law, hereby depose and
state:

1. That I am the legal owner of the parcel of land covered by OCT No. P-7307 and Tax
Declaration # 02-19028-00044 as evidenced by Deed of Absolute Sale bearing Doc. No.
2232; Page No. 93; Book. No. 54; Series of 2010, Notarized by Atty. Vicente G. Rudas
Notary Public of Abuyog, Leyte;

2. That my brother ANTONIO C. CORSIGA is applying for a building permit and

Electrification Permit at the Office of the Municipal Engineer of the Municipality of Javier
so he could build a building on the portion of the abovementioned parcel of land;

3. That I hereby signify my written consent, freely, willingly and voluntarily to allow my
brother to build their residential house on the portion of the said parcel of land and
consequently, have an electric connection on his building;

3. That I am executing this affidavit to attest the truth of the foregoing facts as to my
undertaking to give consent to my brother to support his application for a Building Permit
and Electrification Permit;

4. That this affidavit of consent does NOT give authority to Antonio C. Corsiga to dispose,
transfer and convey the portion of the above-mentioned property where his house is build
and/or the entire property.
