QUITCLAIM AND RELEASE WITH MOTION TO DISMISS

COMES NOW, the undersigned complainant EDGARDO ALBERTO, in the

above-entitled case, before this Honorable Office, respectfully manifests:

That for and in consideration of the sum of FORTY-THREE THOUSAND

NINETY-FIVE PESOS AND EIGHTY-THREE CENTAVOS (P43,095.83) under BDO
Check No. 207456 in full satisfaction of my complaint and/or in settlement of my
claim/claims as financial assistance and or gratuitously given by my employer, receipt
of which is hereby acknowledged to my complete and full satisfaction, I HEREBY
RELEASE AND FOREVER DISCHARGE the above-named respondents from any and
all claims by way of wages, commission, overtime pay, holiday pay, differential pay,
service incentive leave, cash bond, uniform allowance and 13 th month pay or otherwise
as may be due me in connection with my past employment with said respondents. In
addition, I received the amount of Php ___________, for __ sets of uniform that I
returned to the company. I hereby state further that I have no more claims or right of
action of whatsoever nature, whether past, present or contingent against said
respondents.

I hereby affirm that by virtue of this quitclaim and release I have no more claims
or right of action whatsoever nature, whether past, present or contingent against said
respondents. IN VIEW HEREOF, I hereby move for the dismissal of the above-entitled
case with prejudice.

IN WITNESS WHEREOF, I have hereunto freely, voluntarily, intelligently, and

without any mental reservation set my hand this 1 st day of December 2020 in Las Pinas
City.
