    Greetings!

    This comes respectfully in your knowing that the undersigned would like to ask for the approval and
    other relevant actions from your office of the enrollment in the LIS of HADLOCON, NATHANIEL
    PARDILLO with LRN: 228501060412. The said student is a BALIK-ARAL last 2017-2018 due to Financial
    Problem and is currently enrolled in Grade 9-Cabando.

    Attached herewith are the photocopies of the pertinent documents of said student for your perusal.
    However, the undersigned would like to inform that the learner’s SF 137/10 from his previous school is
    not yet available on file currently though the request for the said document was already given last
    August, 2017.
