AFFIDAVIT

I, MARIA LUISA D. CHUA, of legal age, Filipino, married and a resident of

San Jose, Bacon District, Sorsogon City after having been sworn to in
accordance with law do hereby depose and state:

1. That I am the above affiant;

2. That I am the legal owner of one (1) piece chainsaw with model MS
070 (2018) and serial numbers 186672025;

3. That I purchased the said chainsaw from Sorsogon Universal Trading

at Magsaysay St., Sulucan, Sorsogon City on March 16, 2020 with the
purchase price of Sixty-Four Thousand and Eight Hundred Pesos
(Php. 64,800.00):

4. That I am executing this affidavit in order to attest to the veracity of the

foregoing statements and to affirm that the same are true and correct
to the best of my knowledge and belief, specifically, as compliance
for the requirements of the Department of Environment and
Natural Resources (DENR) on the application of permit for said
chainsaw and for all legal intents and purposes this may serve;
