    RESOLUTION REQUESTING LANDBANK OF THE PHILIPPINES, AGOO, LA UNION


    BRANCH TO ENROLL ONLINE PAYMENT OF PHILHEALTH PREMIUM CONTRIBUTION
    THRU ONLINE PAYMENT FACILITY OF THE ELECTRONIC PREMIUM REMITTANCE
    SYSTEM (EPRS) OF BARANGAY TUBOD, SANTO TOMAS, LA UNION.

    Whereas, it is mandated by PHILHEALTH Circular No. 2020-0008;

    Whereas, the Authorizer Punong Barangay HERMAN J. BASBAS and Barangay Treasurer
    TERESITA G. LARON is required to enroll online payment of PhilHealth Premium
    Contribution (ERPS;.

    Now Therefore, on motion of Sangguniang Barangay Member MICHAEL P. PERALTA and


    unanimously seconded.

    RESOLVED AS IT HEREBY RESOLVED, to approve the request to the Land Bank of the
    Philippines, Agoo, La Union branch for the enrollment payment facility o EPRS of Barangay
    TUBOD, Santo Tomas, La Union with Account No. 1392-1013-29 and Barangay PEN No.
    01900025105.
