Doc. No.162, Page No. 34, Book No. 33, Series of 2020, of Notary Public
________________________ dated December 07, 2020.

In accordance with DAR Clearance Requirements, I have attached the following

documents in support of this request per instructions I have received from your office:

1. Letter Request of vendor and vendee;

2. Deed of Sale;
3. Certified Machine Copy of the Title issued by the Register of Deeds;
4. Sketch Plan/ Approved Plan;
5. Certified Copy of Tax Declaration;
6. Tax Clearance;
7. Municipal Assessor Certification as to the total landholding in the name of
Vendee and Vendor;
8. Provincial Assessor Certification as to the total landholding of Vendee and
Vendor;
9. BARC Chairman Certification that his office was furnished copy of the Affidavit of
the transferor and transferee and instrument of transfer;
10. Affidavit of transferor that the property subject of transaction is a retention or part
of his retention;
11. Affidavit of transferee that his/her landholdings, inclusive of the lands to be
transferred to him/her will not exceed five (5) hectares; and
12. Duly Notarized SPA executed by the Vendor
