2. PURPOSE
1. The following is a document of the Quality Control procedures for formwork work for the project.
The objective of this document is two folds:
1. To demonstrate our total quality commitment in implementing, managing and executing of the works for this
project,
2. To assure the client and their project managers and Engineer the confidence that the contract requirements will
be satisfied by the contractor every respect of the works.

3. SCOPE OF WORKS
This document summarizes the general details of the quality assurance plan for system forms as well as traditional wooden
forms including:
1. Management Responsibility
2. Shop Drawings
3. Procurement
4. Fabrication
5. Erection
6. Inspection
7. Striking
Regarding the above procedures, not only the quality aspect of concrete formwork but the work methods will be described to
a relevant extent so that the actual work process can be envisaged.
This document is the beginning, not the end. This document should be communicated reiterated, regularly reviewed and
developed to suit changing condition, standards and technology as the site situation requires

6. RESPONSIBLE FOR QUALITY CONTROL.
1. The Site Manager will be responsible for the concrete formwork works. His duty also includes coordination with
Engineer for inspection prior to concreting work. Or closing the shutter.
2. The QA/QC Manager will be directly responsible for assuring compliance with the contract documents. His duties
further include:
3. Implementation of the Quality control plan to ensure accurate and adequate inspection compliance with contract
documents and direct corrective action upon any occurrence of noncompliance to contract documents.
4. Record all Quality Control activities including inspection and take appropriate measures to correct defective works.

5. The implementation of the Quality control plan will be the responsibility of the Project Manager who will have total
support of the company’s management and staffs for review and approve the all documents and letters related with the
concrete formworks and submitted to ENGINEER.

7. PROCUREMENT & FABRICATION


7.1.Specification Requirements
The following types of form materials have been specified:
1. Formwork: Undamaged plywood or other panel type materials
2. Form ties: Factory-fabricated, adjustable length, removable or snap-off metal form ties
3. Form coating: Commercial formulation form coating compounds

7.2.Shop Drawings
1. Shop drawing showing location of construction joints as well as precise dimensions of the structure form design
and other details will be submitted to Engineer for approval.
2. Only after getting approval, shop drawings will be released to the subcontractors for form fabrication. Sufficient
numbers of copies will be prepared on the site to assure that actual works are undertaken in accordance with the
approved shop drawings.

7.3.General
1. System forms will be used for the larger portion of this project. Conventional plywood forms will be also used in the
sub-structure, stair, and other relatively small or odd parts of formwork. Subjected to the approval of the Engineer.
2. System forms will be procured from the manufactures that are specialized in both design and fabrication of custom-
made forms for the specific projects.

7.4.System Form for Wall “W3” & “W4”


1. The climbing forms for Wall “W3” & “W4” will be manufactured like SGB. The whole load of the system will be
supported by anchors cast in the wall concrete underneath and the system will climb along the climbing walls
connected to the anchor bolts.
2. Temporary scaffoldings will be attached to the system at both inside and outside of the wall. In addition, two stories
of working platforms will be erected on the system to shut inside of the wall and to provide the space for workers
and materials.
3. Durable plywood panels of 18mm thickness, which can withstand repetitive uses, will be used for the surface of
formwork. The plywood panels will be fastened on lattice girders extending vertically, and make up the unified
single piece form panels.

7.5.System Form for Columns


1. System forms for the column will be fabricated by the typical plans and sections of the preliminary design to be
approved. The detailed is currently underway by SGB or equal.
2. The column forms are fabricated and connected on the site using stripping pieces. Durable plywood panels of
18mm, which can withstand repetitive uses, will be used for the surface of formwork.
3. Working platforms with handrails will be attached to the side of the column forms for not only formwork but also
other operations like rebar installation and concrete placement.

8. TEST & INSPECTION


8.1.Site Inspection Plan
1. The exact date and time of form inspection prior to concrete placement or form closing will be informed to as early
as possible. However, because of very tight schedule of the whole project, timing of inspection needs to be flexible.
2. An approved formwork checklist attached will be provided to inspect the form erection. This sheet will be filled by
the staff who is not only performs the internal inspection but also attends the inspection by contractor.
3. Corrective actions on the erected formwork will be directed to subcontractors based on the items on formwork
checklist
8.2.Laboratory Testing Plan
No laboratory testing is required in terms of the work.

9. CONTINGENCY PLANS
9.1.Formwork Hoisting Equipment
One mechanic will be on standby to service any breakdown of the tower cranes. Even if one tower crane is completely
out of operation due to mechanical troubles, another tower crane can serve for hoisting forms, allowing that the
productivity goes down considerably while repairing work takes place.

9.2.Night Time Works


Sufficient manpower and equipment will be provided for all night formwork erection including standby service support
personnel (e.g. tower crane operators, mechanics, electricians, etc.)
Adequate illumination will be provided at all work areas to assure safe and efficient operations during night

9.3.Traceability of Products
Formwork itself is a temporary work after all. Therefore, “traceability of products” will not be directly applicable here.

9.4.Control of Non-conforming Products


1. Plumb and alignment and other aspects of the formwork will be checked preliminarily just before rebar’s installation
and finally before concrete pouring and paid full attention during concreting operation works so that any changes
fall within the specified tolerances.
2. If there occurs slanting or deflection of the formwork over the tolerance range, it may lead to discrepancy of
embedded items location. In this case, an appropriate way to accommodate installation of steel beams or eyebrow
trusses will be determined bases upon duly consultation with Engineer.
