PROMISSORY NOTE

I hereby apply for a Loan in the amount of ___________________________. I hereby swear that:

1. I am up-to-date in the payment of ONE's monthly association dues;

2. I am up-to-date in the payment of the existing loan;

3. I have no pending application for retirement or is not due for retirement within the term of the loan;

4. I have not guaranteed more than 2 OMPLs;

5. I am not a respondent/party of any pending administrative or criminal cases; and

6. The statements declared above are true and correct, and any misrepresentation discovered may cause outright
disapproval of the loan application.

I promise to pay the Organization of NAMRIA Employees the amount received in accordance with the terms and
conditions, which I acknowledge to have read and clearly understood.

TERMS AND CONDITIONS

For the Borrower:

1. I hereby agree that the Loan shall be payable in ___ month(s), in equal monthly amortizations payable through salary
deduction.

2. I hereby agree to pay OMPL's interest on loan at 7% per annum, which will be added to the loan immediately, without
any service fee.

3. In case the salary deduction is defaulted or delayed, I hereby agree to pay the amount due in cash through the OMPL
Chairperson, without furthern need of notice or demand.

4. In case of non-payment and/or underpayment of this loan, I hereby agree to authorize ONE to deduct and amend the
amount of amortization and the number of payments without prior notice.

5. I hereby agree that the stopping of salary deduction shall not be allowed unless a written request is made and approved
by the Business Manager and President.

6. In the event of separation from service for whatever purpose, I hereby agree to settle my unpaid balance in cash. In
case I cannot settle my unpaid balance in cash, I hereby authorize ONE to deduct the unpaid balance from my last salary
and/or separation benefits and thereby waive my rights under applicable rules.

1. In the event that the BORROWER defaulted or failed to pay any of the monthly amortization, I hereby agree to pay the
outstanding amount due, withour further need of notice or demand.
