Subject : Deworming for the school year 2019-2020.

1. Pursuant to DepEd Order #____ s. 20__ also known as _____(title of DepEd memo)______.,
stated that mass deworming of all learners should be done twice a year or every six
months (June an January). On July 24, 2019, the DepEd Camarines Sur will conduct its
1st simultaneous Deworming day for the school year 2019-2020.

2. The program aims to reduce or eradicate the occurrence of SOIL-TRANSMITTED

HELMINTHIASIS WITH FOCUS ON DEWORMING, to maintain healthy and well-nourished
learners.

3. All School / Districts shall prepare and submit the following accomplished forms:
3.1. The Deworming Form #1; this form will be accomplished by the advisers and to
be given to their respective school heads.
3.2. The newly updated Deworming Form #2; shall be prepared by the schools heads
and to be submitted to their respective District offices.
3.3. The newly updated Deworming Form #3; shall be prepared by the District
Health Coordinators or by their District Supervisors and to be submitted to the
Division Health Section.

4. The district Nurse assigned shall supervise the conduction of the Deworming day and
shall coordinate with the school heads and other agencies concerned.

5. All reports should be submitted on or before the 31st of July 2019.

6. Immediate dissemination of this memorandum is desired.
