JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, JOSEFA L. GUZMAN, of legal age, Filipino, married presently residing at Barangay Buag,
Bambang, Nueva Vizcaya and KAREN GAY U. DEL ROSARIO of legal age, Filipino, married and
presently residing at Barangay Homestead, Bambang Nueva Vizcaya, both after being duly sworn to in
accordance with law, depose and state that:

1. We personally know Ms. Leonor Ateo-An of Barangay Buag, Bambang, Nueva Vizcaya, being
acquaintance for a long period of time;

2. We declare, confirm and affirm that she was born at Bontoc Mountain Province Philippines, on
the 16th day of July 1980;

3. She has been using the said birth date in all her documents and transactions;

4. We are not related by affinity or consanguinity to the above mentioned person; and

5. We are executing this affidavit to confirm and affirm the foregoing facts and
for whatever legal purpose it may serve.
