CONSTRUCTION ENGINEERING AND INSPECTION
SERVICES FOR CR 30A BRIDGE REPAIR
the Gulf County Board of County Commissioners, herein referred to as the "County", is seeking
statements of qualifications from Engineering Firms to provide CEI services which are required
for contract administration, inspection, and materials sampling and testing for bridge repairs to
Structure #510046 located on CR 30A in Gulf County, FL. Repairs include guardrail replacement,
installation of fabiform conerete mattresses, approximately 2 tons of rip rap rubble and milling and
resurfacing the bridge deck and approaches. s shall be FDOT qualified in Work
Type 10.1: Roadway CEI.
RFQ DEADLINE: April 1, 2021 no later than 4:00 PM Eastern Time at the Gulf County Clerk’s
Office, 1000 Cecil G, Costin Sr. Blvd, Room 149, Port St. Joe, Florida 32456, and will be opened and
read aloud at this same location on April 5, 2021 at 10:00 AM Eastern Time.

general contractors to Gulf County for ability to submit lump sum. bidding for identified Gulf County
residential homes.
RFQ DEADLINE: RFQ must be marked with “SEALED BID #2021-14” and Titled HLMP RFQ.
Submitted on or before March 19, 2021, no later than 4:00 p.m. Eastern Time at Gulf County
Clerk’s Office, 1000 Cecil G. Costin Sr. Blvd, Room 149, Port St. Joe, Florida 32456, and will be

The County reserves the right to waive minor irregularities, to reject any or all proposals received, and to
otherwise make a decision in the best interests of the County. The Board shall have the final decision
making authority concerning any final award. or interim or comprehensive agreement entered into by the
County. Proposals will be evaluated and ranked in order of preference by a Selection Committee to be
appointed by the County Administrator as authorized by the Board of County Commissioners. The
Selection Committee, at its option, may request that proposers make presentations and/or participate in
questions and answers sessions. The County may also request additional information from proposers that
may be necessary to fully evaluate a proposal. Afier the Selection Committee has ranked proposals and
submitted recommendations to the Board of County Commissioners, the County will then begin
negotiations with the highest-ranked firm.

THIS LEASE made by and between the County of Gulf, a
Political subdivision of the state of Florida, acting by and
through its Board of County Commissioners, hereinafter called the
"county* and the GULF COUNTY SENIOR CITIZENS ASSOCIATION, INC., a
non-profit Florida corporation, hereinafter called the "Center",
whereby the parties for and in consideration of the mutual
covenants and conditions hereinafter expressed, agree as follows

2. ‘The Center agrees to erect the building in a direct
Property line with the Gulf County Public Library with the exposed
part of the facing of the building being brick.
3. The Center agrees to furnish a survey of the property
along with a drawing showing electrical, water and sewer lines to
be obtained.
4. The center will pay to the County as rent for the
leased property the amount of One Dollar ($1.00) per year with the
first payment falling due on the 14th day of Septeuber, 1993,
thereafter like paynenta shall be due and payable not later than

the like day of each succeeding year of the term of this lease598
5. This lease shall, be for a term,commencing the 14th
Gay of September, 1993, and ending the 13th day of September, 2043,
unless terminated at an earlier date in accordance with the
provisions hereinafter set forth.
6. The Center agrees that the leased property will be
used only for the purposes of maintaining and operating a senior
citizens and community center, and the parties further agree that
4£ the County shall make a good faith determination that the leased
property is no longer being used for said purpose or for a valid
county purpose, then this lease shall cease and terminate and be of
no further force or effect. Further, the parties agree that if
there is a change in circumstances that causes the termination of
this lease to be in the best interest of Gulf county, as determined
in the sole discretion of the county, then the County may terminate
this lease provided that the County shall make an equitable
settlement with the center for the value of any improvements made
by the Center to the leased property which cannot be renoved by the
center from the leased property, as the County in ite sole
Glecretion shall determine.
The Center agrees that neither thie lease nor any
portion thereof shall be assigned or sublet without the prior
written consent of the County, which consent may be withheld or
granted in the County's sole discretion. The Center may conduct
evente within the building to be constructed that will provide
funds for the operation of the Senior citizens Center.
8. The Center agrees not to erect any permanent
improvements on the leased property without the prior written
consent of the County. Further, the County agrees that the Center
may remove, upon the termination of thie lease, any inprovemente
placed on the leased property by the Center provided that the
leased property ie returned to the County in substantially the same
condition that the leased property ie on the firat day of this
lease, normal wear and tear expected.
9. The Center accepts the leased property together with
all facilities and improvements located thereon in an "aa is
condition without warranty express or implied and the Center agrees

598to assune full and sole responsibility for the duration of this
lease of all Liability resulting out of the use and occupancy of
the leased property and further agrees to hold the County harmless
from any and all such lability, including reasonable attorney's
feea incurred by the County in defending against such claims, In
addition, the Center shall at all tines during the tera of this
lease maintain in full force and effect policies of insurance
providing Liability coverage with limitations of liability which
shall not be lesa than the anount of Liability to which the County
nay be exposed because of any ‘waiver of sovereign immunity" or
similar law. The initial limits of liability for such coverage
shall be $__5,000.00 per person and $200,000.00 per
ocourrence. The County shall be naned as an additional insured on
all auch policies of insurance and shall be annually provided with
a certificate of insurance reflecting that policies meeting and
conditions set forth above are then currently in force and effect.
