mentioning the Co-morbidity is essential.
 For beneficiaries of NEERI Hospital, we can provide the
certificate in person to eligible candidate.
 Note that one who takes such certificate has to come in
person and sign the register on receiving the certificate.
 No certificates will be handed over to any other person
than the patient. All patients are requested to come
with their medical record diaries for getting the
certificate.
 Also no certificates will be issued to non-NEERI
beneficiaries.
