Mr.

Ahmad Case
Mr. Ahmad, a 40 years old male patient, was admitted to the hospital on March 15 th, 2018 suffering
from distended abdomen, dry and hard stool passed difficultly. He stated, “I haven’t passed stool for
three days, I feel pain in my abdomen, I eat little amount of fiber diet and drink little fluid.”

Here are the NCP results, put the following information into the appropriate column. Beware of

1. Mr. Ahmad, a 40-year-old, has distended abdomen, dry and hard stool passed with difficulty.
2. Encourage high – fiber diet intake.
3. Constipation related to little amount of fiber diet and drink little fluid as manifested by
distended abdomen, dry and hard stool since three days.
4. Instruct the patient to limit activities.
5. The patient will pass bowel movement by March 16th 2018.
6. Encourage the patient practice exercises.
7. Encourage and offer oral fluid intake every two hours to the patient.
8. By 16 April 2012 the patient has passed bowel movement with little amount of pain.
9. The patient drank orange juice about one liter, water and some soup about 1.5 liters.
10. The patient ate high-fiber diet.
11. The patient states “I don’t pass stool since three days, I feel pain in my abdomen, I eats little
amount of fiber diet and drink little fluid.”
12. Encourage fluid intake up to 3 liters per day.
13. The patient practiced some exercise as walking.
14. Encourage low caloric diet intake.
15. Assess every four hours the patient’s oral temperature.
