Your are advised to visit this office on any working day along with following Document .
1. Discharge certificate of your late father/husband/Identity Card.
2. Photocopy of Bank’s Passbook and in original .
3. Non Employment/ Re-employment certificate of your late husband/Father.
4. Certificate regarding drawl/non-drawl of any other pension/family pension.
5. Marriage/ Non-Marriage certificate form village Sarpanch.
6. Two latest photographs duly attested by Gazetted officer/Soldier Board.
7. Option for drawl of pension through Bank under NBS is enclosed signed by any one of public
sector bank of list enclosed, from where monthly pension is desired to be drawn. (For list of Bank
Branches P.T.O)
8. Please bring with you two guarantors who are well known to you, for witness drawing pension from
this office . As and when you turn up. Please report only to the officer-in-charge of DPDO and not
to any lower functionaries.
9. An affidavit duly attested by first class Magistrate (Specimen attached)
10. Photocopy of your Aadhar Card (mandatory)/ Voter Card/PAN Card/ECHS Card(mandatory) and
original also.
11. A certificate from village Sarpanch that your husband/father was not re-employed in any state
Govt/Semi Govt/Organization/Bank after discharge from Army Service.
12. D’ Roll duly completed and attested by DSWO of your Dist. (Specimen attached)
