As one of the requirements in the subject Ed. 8 (Assessment of Student Learning 1), I would like
to request approval from your office to allow the following Third Year Bachelor of Elementary
Education students namely: Nicole Grace Muleta, Criselle Verano and Gerielyn Joy Naza of
Iloilo Science and Technology University, Miagao Campus to conduct an assessment among the
Grade IV pupils in your school on October 06, 2017 (Friday) at exactly 3o’clock in the afternoon.

May I also request the Resource Teacher/s be assigned to the students during the assessment?
