AFFIDAVIT FILED ON BEHALF OF THE 1st PETITIONER

I, U.Prashanthi, W/o U.Venugopal, aged about years,
Hindu, residing at
do hereby solemnly affirm and state as follows:-
1. I am the deponent herein and 1st petitioner in the above O.P.
Myself and other petitioners have filed the above O.P. against the
respondent claiming compensation for the death of my husband who
died in a road accident. This Hon’ble Court was pleased to pass an
award on 06-09-2011 for a sum of Rs.3,28,000/- with interest at
7.5% p.a. from the date of petition till the date of realization.
In the award amount I have allotted Rs.1,48,000/- towards my share
and in that I have been permitted to withdraw a sum of Rs.40,000/-.
I submit that the respondent / corporation had deposited only a sum
of Rs.25,000/- before this Hon’ble Court on

2. I submit that hence the present petition seeking a cheque for

Rs.25,000/- as part-satisfaction.

3. I am having S.B. account in State bank of India, Puthalpattu

Branch vide S.B.A/c No.11507208389 and the pass book is enclosed
herewith.
4. I therefore pray that this Hon’ble Court may be pleased to
issue cheque for a sum of Rs.25,000/- (Rupees twenty five thousand
only) infavour of the Branch Manager, State Bank of India,
Puthalpattu Branch with a direction to credit the same into my S.B.
Account No. 11507208389 by enabling me to withdraw the same, or
else I will suffer great loss and damage.
