We, the undersigned incorporators, all of legal age voluntarily agreed to form a religious society
under the laws of the Republic of the Philippines.

THAT WE HEREBY CERTIFY

First: That the name of this society shall be

Bayombong United Methodist Church, Bayombong, Nueva Vizcaya Inc.
Second: That this religious society is a religious corporation affiliated to

Third: That two-third(2/3) of its membership have given written consent and voted to
incorporate during a meeting of the society.

Fourth: That the incorporation of this religious society is not forbidden by the competent
authority or by the Constitution, rules, regulations or discipline of the religious
denomination, sect or church to which it forms part.

Fifth: That the purpose of this religious society is

To administer, hold and manage its affairs, temporalities, properties and
revenues in trust with the United Methodist Church for the use, objectives,
activities, ministries, on behalf and for the benefit of Bayombong United
Methodist Church, Inc.; to operate an educational institution; and do all lawful
acts, and things to enhance, sustain, protect, develop and promote the faith, best
interest and welfare of Bayombong United Methodist Church, Inc. and its
congregation.
