My child/ward will comply with his/her clearance once the Enhance Community Quarantine is

lifted and office operation normalizes.

I voluntarily agree that should my child/ward failed to comply with his/her clearance

requirements FEU High School is not obligated to accept him/her as their regular student. I

further hereby voluntarily agree that I free FEU High School from legal and educational obligation

towards my child/ward should this agreement is not complied.
