Introduction

Introduction including overview of area inspected and activities taking place


This report is about the identification of the hazards and potential risks of safety which were
present during the visit of Binham Steels Works LLC at 05 August 2019. The company is
located at industrial area opposite Mubarak market Al Ain UAE. The number of workers in
different fabrication trades is 50. The work starts at 9 am and ends at 5 am. This means those
normal working hours of the COMPANY is 8 hours. Overtime is done according to the
requirement of work.
The metallic gate which is 12 fit wide is the main entrance of the workshop. The place of parking
is in front of the main gate of the workshop. In case of emergency, the parking area is also used
for storage of material. Some of the areas of the yard are covered by GI corrugated sheets
which are installed over a structure made of steel.
There are 2 halls in the workshop which are at the right and left the side of the main entrance
gate. Hall number 1 which is on the right side has Bar Bending Machine and a sheet cutting.
The left half is used for the fabrication work. It also contains bench grinder, power press,
welding plant, pedestal drill, gentry crane and other tools of fabrication. The bricks that are used
in the construction are red for extra strength.
All the operations are done at ground floor. It is a single story workshop with a little roof. This
mini roof is used for metallic scrap. This area is inspected and identifies the potential hazards.

Executive Summary

After the complete inspection, I found the various areas of concerns. The sanitary convenience
and ventilation are in good conditions and the workers are feeling satisfied. These facilities are
according to the number of employees.
The concern areas posed potential hazards and risk to the human life. In serious situations,
these risks can lead to the death. In addition, these potential risks require the immediate
attention of the owner or management. There is the guarding on machinery like a bench grinder,
power press, and sheet cutting
Machine. The conditions of the machinery are not appropriate. There is no uniform for the
operators who are too close to rotating parts such as power press. The loose clothes are also
an additional hazard for the workers. In addition, the condition of wiring is not suitable for the
workers. Naked wires are the alarming situation and can cause a big problem. This situation is
signing about the pose of danger/hazard electrocution and fire. The other problem is that there
is no emergency exit. The only exit is towards the car parking at the main entrance gate. The
condition of wiring is showed that it does not maintain or repair for a long time. It may be done
by poor and untrained electricians. There is also lack of the safe electrical wiring techniques.
These naked wires are visible in entire working halls. Workers have also lacked workplace
safety awareness because some are hanging the clothes on naked wires. There is no concept
of the Personal Protective Equipment (PPE) with the workers. This is the negligence on the part
of the management. They instantly pat attention to resolve this issue. Workers also have lack of
the knowledge about the protection and workplace hazards. According to the nature of hazards
like tripping, noise, striking against steel parts, radiations from welding, steel parts falling from
height etc., the Personal Protective Equipment (PPE) is necessary to provide in the workplace.
Fire equipment such as secondary exit and fire extinguishers are not present. All the work is
done with manually in the workshop. This may lead to serious musculoskeletal issues.
Most of the observations can be rectified within few months and cost is also feasible and
bearable by the management. There is also need to invest in the training and development of
the employees so that any kind of unseen incidents may be minimized or control.

Main findings of the inspection


Permanent Disability-Machine Guarding:

The machines used in the fabrication purpose are without the rotating parts such as pulley or
belts without the guard or cover. This is a serious threat for the workers in the workshop. On the
other hand, the operator is seating too close to the power press machine’s exposing parts which
were the pathetic situation. There were chances of entanglement with the pulley due to lose
clothes wear by the machine operators. It also has the potential for severe head injury. The
behaviour of the operator was also very neglecting while operating the machine. He was also
listening songs which cause loss of concentration. Any little mistake can easily crush his arm
due to power press. The case was the same with other machines. Abrasions to fatality and
minor cuts are resulting due to an accident. On the other hand, the cost of the insurance claim is
higher due to these types of accidents. Further, the owner has to bear the subsequent fine,
treatment & clean-up cost, loss of production due to accident and penalty which likely to run into
thousands of dollars. On the other hand, the loss to the victim’s family cannot be measured in
monetary terms.
The current situation of the workshop is a direct breach of the Article 16(1) of the ILO
Occupational Safety and Health Convention C155. Under this article, there is needed
equipment, processes and workplace machinery to be safe and without the risk to the health of
the workers. In addition, this is essential to guard all the rotation parts of the machine.
The instant solution to avoid accidents in this situation is to relocate the position of the power
press from the pulley and belts. The operator strictly bound not to wear the loose clothes while
working on the machine. In addition to this, install the fixed guard with locks on all pulleys,
arrange to fabricate, and unsecured rotating parts of machines in the workshop. Other than the
maintenance person, the removal of the guard should prohibit. Installation and fabrication of
these guards cost no more than 1200 AED and can complete within a month including the other
procedures such as obtaining quotations, material delivery, installation, painting, fabrication, and
delivery of material. Hence, periodical inspection and maintenance is necessary to reduce the
risk.

Electrical Hazards-Electrocution:

The problem of the electricity started right from the entrance of the main gate. The thick electric
cable is tied to the metallic gate which is a real life-threatening hazard. It has potential to cause
electrocution in case it accidentally energized. Anyone moving in or out can meet this gate. In
the workshop, there are naked and loose wires at different places. The naked wires are also
running to the breaker naked which is behind the sheet cutting machine. In addition, the
situation is worse as clothes are hanging beside the breaker. The fire can spread if there is
spark even due to overheating. In no time, the clothes can catch fire and cause serious damage
to the workplace as well as to the workers. This may turn uncontrollable because there is no
firefighting equipment install. Plug-less wires are inserted into a broken extension board which is
hanging loosely. This is an open invitation for the electrocution and electric shocks. In case of
an accident, this can be a serious problem for the owner, workers, and also for image and
reputation of the factory.

The solution to handling this situation is that immediately removed the naked wires and installs
the new ones. There is need to shut down some operations for some time. This is not a big loss
of shutting down the operations. Properly insulated wiring either in channel duct or concealed
may be installed. In addition, the wiring should be in international and quality safe standard. The
estimated cost for this change is not more than 1100 AED. It is the duty of the management to
hire the trained electricians for the wiring and the maintenance of electric wiring. The training
may be arranged by hiring the local Electricity Inspector. This will cost only $150. This is a very
little cost which will incur on an electrician but the benefits received from it will save millions of
dollars for the business and the management. The reason is that the company not has to pay
premium or insurance claim. This will create the sense of the secured environment among
workers and increase the confidence of electricians.

Disability or Injury- Personal Protective Equipment (PPE):

A number of operations are performed during the production of a single unit. Several machines
are also working and operating in the workshop such as hot job, drilling, cutting, hammering,
machining and bending. Every corresponding job has its potential risks or hazards like falling of
objects from a height, vibration, noise and striking against the metallic material. It is the big
responsibility of the management to search for the solutions proactively to handle these hazards
like reduction, elimination, reengineering, and isolation and administration actions to control
hazards. All these things are easily manageable with help of the proper planning. Management
should provide the Personal Proactive Equipment (PPE) to the workers and provide them
training to use this equipment in time of need or emergency. Lack of awareness and
understanding is evident in workers about the PPE. In addition, there is no equipment or
earphones are available for the workers to reduce the high noise of hammering in the workshop.
On the other hand, welders are welding with naked eyes, and directly exposed to the welding
radiations. The immediate impact is on the eyesight of the welders. Spectacles are a simple
solution to avoid the risk of weak eyesight. However, the workers in the workshop have no
knowledge about PPE which makes the situation more critical. Even they are not familiar with
the workplace health and safety. In addition, there are no shoes available for the workers
working on the metal, oil spillage or trailing cables.
This is also a clear violation of ILO Occupational Safety and Health Convention C155 on the
part of the management. It is the duty and responsibility of the employer to provide the safe
workplaces for the employees.

In addition, according to the working situation, it is recommended that management should


immediately provide the welder with welding sheet, welding helmets, gloves and also terminate
the welding work until the job is done about providing the appropriate welding equipment. The
cost which will occur on purchasing of the welding safety equipment is about 250 AED. In
addition, there is also need to arrange the training session for the welders about the safe use of
the equipment and advance welding techniques. On the other hand, there is also need to
implement and develop PPE Matrix in the workplace at a suitable place. The per annum cost for
purchase of PPEs is approximately 2100 AED.

Fire Hazard-Multiple Deaths/Burns:

Fabrication work also included the Gas Tungsten Arc Welding (GTAW) also called argon
welding and electric welding or Shielded Metal Arc Welding (SMAW). Both kinds of the welding
produce spatter which may cause a fire if encounters cloth, paper waste or oil. The fire can also
cause by the non-standard and naked wire in the workshop. The situation is too dangerous as
the oil is spilled on the floor from the bar bending machine and the presence of argon cylinder
near it. In addition, there is lack of the control mechanism for chemicals or oil spills. In addition,
there is also no firefighting equipment in the workplace. The worker has no idea about the
severity of the situation and the unsafe environment they are working. In case of the fire
incident, there is no fire alarm to alter the workers and evacuate from the place. Also, there is no
hydrant or fire extinguisher is available on site. The only exit is from the main entrance and into
the parking yard. In case of rush hours, it is often blocked by the motorcycles and the car
parked in the yard. The people working there are at the most of the risk and also the visitors
who have no induction training and evacuation plan. The fire hazard can be costly in millions for
the business in terms of insurance, instruments, customers, and production.
In addition, the management has to immediately provide the firefighting equipment, sand
buckets, and proper training to the workers to handle the situation. The sand buckets are no so
costly. There is also need to provide the high-quality fire extinguisher cylinders in the workplace.
At least two CO2 & two dry powder type fire extinguishers of 3 kg would place in the suitable
areas in the workplace. The numbers can increase according to the requirement. There shall
also provide the fire exit or emergency exit other than the main entrance. The costs of fire
extinguisher will less than 1500 AED. In addition, the employee shall also provide the training.

Manual Handling Risks:

The machines are heavy in nature and workers are operating them manually. This is a big threat
to their physical health in long run. The manual handling of the steel sheet rolls and other
fabricated items are quite normal and frequent in the workshop. One reason is that there are no
proper tools to shift the rolls from one place to another. Due to this reason, workers are forced
to manually shift the items in which require them to interpret the awkward bending, stooping,
poor posture, lifting and twisting of the trunk. This may lead workers to musculoskeletal
problems such as crushing injuries, sprain, rupture of intervertebral discs and strain. In extreme
situation, some of these issues become irreversible and may cause the permanent disability.
It is suggested to stop the manual handling of heavy items immediately like sheet rolls.
Management ensures to provide a couple of sack trolleys for the moving of the metallic items.
The castor wheels are required to purchase from the market, rest of things can be fabricated
using the in-house resource. The time for this is not more than a week. The cost of material and
labour will 1800AED.

Conclusions
The provision of welfare facilities to the workers are provided in the workplace like changing
clothes and toilets. However, these are not enough to reduce the potential hazards there. The
above-mentioned causes of incidents should be addressed immediately in order to avoid any
serious problem or issue. Specifically, the guarding of machinery is essential so that risks may
reduce. It has been concluded that management also has lack of interest in the workshop
safety. Fire is another big threat which requires immediate attention otherwise this can lead to
the big loss to the business in long run.
Proper PPE should also provide the management to stop the manual handling of heavy items.
This is a chronic issue for the workers. The problems are direct shows the violation of the
international and national safety standards and legislation. In case, the management is not
paying attention to these problems then ILO Occupational Safety and Health Convention must
attract for the legal proceedings like case, fines, bans, imprisonment (in extreme situation) and
penalties. However, these will also lose the brand image, business reputation and goodwill of
the business.
On the other hand, it has been suggested that the management should follow the
recommendations which are outlined in this report and action should be taken according to the
timeline provide.

It is the responsibility of the management to commit to health and safety of the workplace and
employees also as legal compliance. This is the first step of management to follow the
standards describes by the law. These things ultimately increase the worker's motivation,
morale, and satisfaction, secure the workplace, reduce the incident rate and provide a safe
environment to the workers. In addition, the management should also introduce the proactive
safety culture.
