    I hereby give my consent for the above named participant to participate in programs
    through TNT All Stars Inc. I realize that such activity involves the potential for injury,
    which is inherent in all sports. I acknowledge that even with the best coaching, use of the
    most advanced protective equipment and strict observance of the rules, injuries are still a
    possibility. On rare occasions these injuries can be so severe as to result in total
    disability, paralysis, quadriplegia, or even death. I agree to exonerate and serve harmless
    TNT All Stars Inc., it’s agents, servants, employees, and volunteers, including coaches,
    trainers, and all other practitioners of the healing arts treating my son/daughter, from any
    and all liability, claims causes of action or demands of any kind and nature whatsoever
    which may arise by of in connection with my son/daughter’s participation in any
    activities related to the sport indicated above. I understand that I will be responsible for
    payment according to the following schedule: There is a one time yearly registration fee
    of $15.00. ALL PAYMENTS ARE DUE BY THE FIRST DAY OF CLASS EACH
    MONTH. A $15 LATE FEE WILL BE ADDED TO ALL PAYMENTS AFTER
    THE 5TH OF EACH MONTH. Any payment over 21 days late the participant will not
    be able to participate at TNT All Stars. Any payment over 30 days delinquent will be
    charged an additional 18% per annum. Furthermore, I understand that the children will
    be participating in group competitions that end each year in October/November for Little
    League participants and that early termination creates a hardship on the team.
    Accordingly, I agree to pay an early terminations fee of $100.00, in addition to any
    outstanding fees due on the Contract, if the participant withdraws prior to the end of
    October/November for Little League participants. I also agree to pay any and all attorney
    fees and costs incurred in collecting delinquent payments.
