Joint Affidavit
I, VINCENT C. ANTIDO and JOENALDO P. HERMO, of legal ages, Filipino Citizens
and residents of Iligan City, after having duly sworn to an oath as required by law, hereby
depose and say:

1. That we personally know the person of MARITES PADING LOYOLA, born to Erlito
S. Pading and Teodora P. Catubig on December 15, 1965 at Misamis Oriental;

2. That the said MARITES PADING LOYOLA has been confined at Gregorio T. Lluch
Memorial Hospital, Pala-o, Iligan City since March 28,2021 until present;

3. That in the hospital record of the said MARITES PADING LOYOLA, her date of birth
has been inadvertently entered by the informant (common-law husband) as
“AUGUST 1, 1966”, instead of the correct one which is “DECEMBER 15, 1965”;

4. That either the names be, MARITES PADING LOYOLA born December 15, 1965
or MARITES PADING LOYOLA with erroneous date of birth appearing in hospital
record which is “AUGUST 1, 1966”, refer to one and the same person;

5. That we know the foregoing facts as we are longtime neighbors and colleagues of
the said MARITES PADING LOYOLA;

6. That we are executing this affidavit to attest to the above facts and for such other
legal purposes it may serve.
