JOINT AFFIDAVIT

We, Jaylucky Sandi and Tara Ann Sandig, of legal age, married, Filipinos and residents
of Brgy. Himamanay, Balasan, Iloilo, under oath depose and state that:

1. On February 21, 2016, at around 5:00 in the afternoon, I, Tara Ann Sandig and our
son Mervin Sandig, 13 years old, were about to travel from Balasan, Iloilo to Roxas
City, Capiz where I, Jaylucky Sandig was waiting for them since we need to buy
school supplies for the project of Mervin.

2. While waiting for a Ceres Bus or an L300, I, Tara Ann saw my cousin Lyndon
Billones on board a motorcycle. He offered my son and me a free ride.

3. While we were already in Pilar, Capiz, I, Tara Ann decided to let my son Melvin ride
a Ceres Bus or L300 van because it is difficult for the three of us on board a
motorcycle. I, Tara Ann gave Mervin ₽60.00 for his fare

4. An L300 van passed by and I, Tara Ann hailed it. The van stopped and Mervin
boarded and I, Tara Ann told him that we just meet in Lawaan, Roxas City where I,
Jaylucky was waiting for Tara Ann and Mervin. I, Tara Ann took note of the plate
number of the van which is ABM 5454.

5. I, Tara Ann and my cousin Lyndon arrived at Lawaan ahead of the van boarded by
Mervin.

6. Few minutes after, the van arrived. I, Tara Ann asked Mervin when he got off from
the van where the change of the ₽60.00 I gave him is. He answered that the driver did
not give him any change. I, Tara Ann told him that the regular fare is ₽50.00 but the
discounted fare is ₽40.00 because he is a student. He reiterated that he was not given
any change.

7. I, Jaylucky saw my wife Tara Ann talking to Mervin. I approached them and asked
what happened. I, Tara Ann said that the driver of the van which Mervin boarded was
overcharging and she gave me the van’s plate number ABM 5454.

8. I, Jaylucky borrowed the motorcycle driven by Lyndon so that I can go to the

Terminal located at Pueblo de Panay, Roxas City to make a complaint to a traffic
enforcer against the driver.

9. When I, Jaylucky arrived at the Terminal, I went to Traffic Enforcer Cesar Zaguirre
to make a report. While I was making a complaint with Mr. Zaguirre, I spotted that
the van with plate number ABM 5454 was in the Terminal so Traffic Enforcer
Zaguirre confiscated the Driver’s License and issued a Traffic Citation Report against
the driver. A Copy of the Driver’s License is attached as Exhibit A and a copy of the
Traffic Citation Report is attached as Exhibit B.

10. I, Jaylucky learned that the driver is Glenn Velado Buni. Instead of asking for
forgiveness, he was blaming the conductor for the overcharging of fare. I, Jaylucky
could have forgiven and would no longer pursue this complaint if he said sorry. After
all, I am also a driver myself. But he did not do so.
 11. We are executing this affidavit to file a formal complaint at the Land Transporation
Office for overcharging against Glenn Velado Buni and for other legal purposes this
may serve.

IN WITNESS WHEREOF, We have hereunto set our hands this 22 nd of February 2016 in
