    I Muhammad Hassan currently working in Ministry of Interior as Section Officer BPS-17 would
    like to request for Medical Reimbursement of the expenses incurred on the treatment of my
    father in PIMS, who has chronic heart disease, as specified in rule 2(d) of the Federal Services
    Medical Attendance Rules, 1990. Details of medical expenses along with cash memos, duly
    verified from all the concerned authorities is attached below.
