Learning Activities:

1. What is the role of the Nurse during Infertility Counselling? Enumerate at least 4 and
EXPLAIN (except for Monitoring V/S, Giving meds as ordered, Notify the Physician)

 Extend emotional support and encouragement


Nurses should first provide support and encouragement to the couple and explain to them that
there are many options to go with and this is not the end. Nurses should also make the couple and
the family feel that they are not alone and that you are with them in this journey.

 Assess couple and determine best approach


Different couples may have different actions and perception towards being in an infertility
counselling, thus, the nurse should first assess the couple and match the couple’s pace in going
through treatment options. This role also includes assessment of couple’s knowledge and
eliminating misconception regarding infertility and its treatment options.

 Explain every single possible option, including its pros and cons
Knowledge on the different treatment options will help the couple make an informed decision.
As their nurse, you should present accurate information that they will easily understand and take
into heart. No bias on a certain procedure must be evident in educating them. Information should
be laid clear, including the procedure’s pros and cons.

 Educate client on treatment management


By the time the couple has come up with a decision, the nurse then should proceed to informing
the couple of how the procedure will go through for the next couple of weeks or months. This
includes setting future appointments, financial management, differentiating medication,
educating them on medication administration, informing them of different adverse effects,
making sure they realize the importance of taking medication on time, and securing them that
they are not alone in this treatment journey.

2. Infertility procedures with pictures and explanation.

a. In-vitro fertilization–embryo transfer (IVFET)

IVFET is a method of
assisted reproduction
where the egg cell and
the sperm cell of the
couple is extracted and
fertilized outside the
human body and on a
laboratory dish. One or
more fertilized eggs
(embryos) may be
transferred to the
woman's uterus, where
they may implant in the
uterine lining and
develop.

b. Gamete intrafallopian transfer (GIFT)

GIFT is a modified version of IVF where


through laparoscopy, collected eggs and
sperm will be placed in the fallopian tube
instead of the typical IVF where it is
placed in the uterine lining. This way, it
is considered that the fertilization actually
occurs inside the body.

c. Zygote intrafallopian transfer (ZIFT)

ZIFT somehow combines


the principle of IVF and
GIFT. The gametes, the
egg cell and the sperm
cell are both collected and
combined in a laboratory
dish, following the
principle of IVF.
However, it will not be
implanted on the uterine
lining but rather on the
fallopian tube, with
expectation that it will be
carried into the uterus and
attach itself and develop.
This is usually done when
there is a blockage in the
fallopian tube, preventing
movement of egg or zygote. This procedure will also be done through laparoscopy.

d. Intracytoplasmic sperm injection (ICSI)


ICSI is similar to the traditional IVF. The
determining difference is that in IVF,
millions of sperm cells are placed with the
egg cell, with fertilization occurring when
a sperm successfully enters the cytoplasm
of the egg. In ICSI, a tiny needle, called a
micropipette, is used to inject a single
sperm into the center of the egg. The

embryo grows in the lab for a few days before implanting it on the woman’s uterine
lining.

e. Embryo donation
In embryo donation, also called as third-party reproduction, the couple will be receiving

an embryo that has no genetic connection to both of them. The fertilization occurred
outside the woman’s body through IVF with the sperm and egg cell of two unrelated
donors. This then will be implanted either on the uterine lining or fallopian tube,
depending on circumstances.

f. Intrauterine insemination (IUI)

IUI is a less invasive procedure


and can be done without fertility
drugs. The procedure involves
transferring concentrated semen
to the uterus of the woman
through a thin catheter in the
hopes that fertilization occurs.

Schedule is important and IUI is usually done in conjunction to the woman’s ovulation. The
procedure can also be repeated until an embryo is formed.
