Definition

Modified release dosage form

A modified-release dosage form is a formulation in which the drug-release characteristics of


time course and/or location are chosen to accomplish therapeutic or convenience objectives not
offered by conventional dosage forms such as solutions, ointments, or promptly dissolving
dosage forms.

Modified release drug products

The term modified release drug product is used to describe product that alter the timing and/or
the rate of release of the drug substance.

Classification

There are 4 types of Modified release drug. They are –

▪ Sustained release
▪ Extended release
▪ Prolonged release
▪ Delayed release

Release pattern from the formation/dosage form

The release pattern from the formation/ dosage form is given below –

➢ Once the drug starts releasing the drug by zero order kinetics, the concentration the
plasma constant and remains above minimum effective concentration (MEC) but

below maximum toxic concentration (MTC) for plasma drug concentration v/s time
profile. This will ensure prolonged duration of drug therapy.
➢ Once the drug release starts by zero order kinetic, over the time, the concentration of
drug in plasma starts reducing but it remains above MEC and below MIC.

Typical types of Modified Release Drug Delivery System

The typical types of Modified Release Drug Delivery System (MRDDS) are considered as 3
types. They are –

• Membrane Controlled System – Coated with polymeric


membrane.
• Osmotic Pump Delivery System – Less commonly used types of
modified release system.
• Matrix Controlled System – Formation of mesh like structured
throughout the dosage form.

Advanced types of Modified Release Drug Delivery System

There are 5 advanced types of Modified Release Drug Delivery System. They are –

• Nanotechnology
• Inserts/ Bot system
• Hydrogels
• Multiarticulate Systems
• Bilayer Tablets

Challenges for Modified Release Drug Delivery System

Challenges for Modified Release Drug Delivery System is given below –

✓ The IV drugs having low water solubility & Low permeability


✓ Drugs cannot be used as modified release system.
✓ Always advised to use solubility enhancers & permeability enhancers in given dosage
form.
✓ Larger Dosage strength should be avoided since increasing the dosage strength the size
of the dosage form.
✓ When polymers are used for forming membrane controller, their concentration will
decide the release rate of drug into blood flow.
✓ It is always advised to used the design of experiment tool to get productive results.
✓ For any dosage form obtaining in vitro in vivo correlations are major concern. The same
case is for modified release systems.
✓ Ideal therapeutic index for API should be greater than 10 to form modified release
systems.

After all of this circumstances, we can say that there are only 2 types of drugs are available in
Bangladesh – Gliclazide & Trimetazidine Dihydrochloride.

Conclusion

Though there are many challenges in forming modified release dosage forms but it is definitely
a better choice as part out novel systems for drug delivery. The concept of Modified Release
Formulations emerged with an objective to improve patient’s compliance. Usually this is to
slow the release of the drug and keep steadier levels of the drug in the bloodstream so that the
medicine doesn't have to be taken too often and therefore improves compliance.
