Nsg diagnosis: hypertension related toelevated blood pressure asmanifested by

occipitalheadache
Interventions:
1. Assess underlying condition
R: To determine what triggers the elevated BP

2. Monitor blood pressure for every 30 mins


R:Changes in BP may indicates changes in patients status requiring
prompt medical attention

3. Observe skin color, temperature, capillary refill, and diaphoresis


R: Peripheral vasoconstriction may result in pale,cool,clammy skin,with
prolonged capillary refill time

4. Do frequent positioning to the patient


R: It may decrease peripheral venous pooling that may potentiated by
vasodilators and prolong sitting or standing

5. Administer prescribed drugs (anti-hypertensive)


R: Inhibits influx of Ca ion across cell membrane to produce relaxation
of coronary smooth muscle

6. Encouraged pt to decrease intakes of caffeine, cola ,and chocolate


R: This are cardiac stimulant and may adversely affect cardiac function

7. Emphasize the concept of controlling hypertension rather than curing it.


R:Hypertension is life time illness, controlling it is the best way to stop it
from reoccurring

8. Encouraged patient to maintain low salt low fat diet


R: Salty and fatty food are one of the common cause of hypertension
