Your claim for the benefits from Pandemic Unemployment Assistance was filed on April 22, 2021.

Determination

You meet the monetary eligibility requirements to establish a claim for Pandemic Unemployment
Assistance benefits. You are entitled to receive benefits as long as you are not disqualified or held
ineligible for other reasons.

Claim Summary

• You are entitled to file for benefits beginning from December 13, 2020, if otherwise eligible.
• You may receive up to $66.00 for each week you are eligible for the benefit.
• You are eligible for up to 79 weeks of benefits.
• Your maximum benefit amount is $5,214.00.

Reason for Determination

In this monetary determination, PRDOL awarded you the minimum weekly benefit amount because we
do not have any proof of earnings/wages in our files. In order to receive a weekly benefit amount higher
than the minimum, you may submit proof of earnings/wages at any time during the Pandemic Assistance
Period to substantiate wages earned or paid during the base period. PRDOL will issue a monetary
redetermination if it concludes the wages documentation is sufficient to permit a re-computation. This
monetary redetermination will apply retroactively to all weeks of unemployment you filed during the
Pandemic Assistance Period. PRDOL will recalculate the weekly benefit amount for any weeks
previously paid and provide supplementary payment as appropriate.

PUA Weekly Benefit Amount Calculation

The PUA rate is calculated based on Puerto Rico’s regular unemployment insurance (UI) formula.
PRDOL takes into consideration all of the claimant’s wages/earnings in the base period or taxable year
completed immediately before the Pandemic Assistance Period. If the claimant has insufficient
employment and wages/earnings in the base period or if the claimant is a self-employed individual who
has no net earnings during the regular UI base period, the PUA rate will be 50% of Puerto Rico’s average
weekly UI benefit amount. Currently in Puerto Rico, the minimum PUA rate is $66.00 and the maximum
rate is $240.00.

The maximum benefit amount shown above does not include any Federal Pandemic Unemployment
Compensation (FPUC) that you are potentially eligible for. Individuals receiving Pandemic Unemployment
Assistance benefits may also be eligible to receive an additional $300 weekly benefit amount under the
FPUC program between the weeks ending January 2, 2021 and September 4, 2021.

You do not need to contact the Department of Labor and Human Resources (DTRH, Departamento de
Trabajo y Recursos Humanos) or submit a separate application to be considered eligible for the
additional $300 FPUC weekly benefit amount. It will be automatically applied to your payment for
qualifying weeks.

Subsequent Claims and Weekly Certifications

You must certify your eligibility every week to continue receiving benefits. Failure to certify each week
will result in non-payment for that week.

Online: To certify online, visit https://puapr.trabajo.pr.gov and log in to your account. Your online account
is accessible twenty-four hours a day and seven days a week.

By Phone: To certify by phone, call (787) 945-7900, Monday through Friday between 7:30 AM and 3:30
PM AST.

Appeal Rights

If you disagree with this determination, you have the right to file a request for reconsideration or appeal
within 15 calendar days from the issue date of this determination.

To file a request for reconsideration or appeal online go to https://puapr.trabajo.pr.gov and log into your
account. Select the “More…”tab and click on the “File an Appeal” link.

You will be asked for a Letter ID that can be found at the top right-hand corner of the decision you are
requesting reconsideration or appealing. Enter the reason for which you are requesting reconsideration
or appealing in the available text box and submit the request.

If you are unable to file your request for reconsideration or appeal online, please call (787) 945-7900 for
assistance.

If you are still unemployed while waiting for your request for reconsideration or appeal to be resolved, you
must continue to request benefit payments each week. Even if your initial determination is reversed, you
will not receive benefit payments for weeks you have not claimed.

Penalties and False Statements

It is against state law to intentionally make false statements or conceal material information in an attempt
to gain payment of unemployment benefits. Individuals filing for benefits fraudulently are subject to
repayment of amounts received, penalties, and interests, and may be subject to criminal prosecution.
