Exhibit “B-4” – Notice sent to the Civil Registrar General, Philippine Statistics Authority
(formerly National Statistics Office)

Exhibit “C” – Affidavit of publication of the Notice of Hearing, that it was published in three
(3) consecutive weeks specifically on___________________, which is not within 30 days prior to any
election nor within four (4) months before the hearing of this case;
