2 A 54-year-old male is brought into your emergency department by his family with
sudden onset of right sided weakness of the arm and leg which started an hour
before. He also has difficulty talking with right facial droop. His BP is 190/110.
a) Describe the major arterial blood supply of the brain. (8)
b) What fibrinolytic drug may be indicated? (1)
c) Describe the dose, indications, mechanism of action and pharmacokinetics
of labetalol as it may be used in this patient. (8)
d) Describe the main factors influencing the regulation of cerebral blood flow. (8)

3 An elderly man is brought into the emergency department complaining of chest pain.
a) Draw and label the action potential of a cardiac cell. (4)
b) Tabulate the categories of the Singh Vaughn-Williams classification of anti-
arrhythmic drugs and give an example in each category. (9)
c) With the aid of a diagram, write short notes on the arterial supply of the heart.

4 While on duty in the emergency department you see a patient with a severe crushing
wound of his right hand and wrist.
a) You decide to provide analgesia using a forearm ultrasound nerve block (FUN
block). Draw a cross-sectional diagram of the right mid-forearm illustrating the
important relations (adjacent structures) of the radial, ulnar and median
nerves. (8)
b) Write short notes on the pathophysiology of the reperfusion syndrome and
myoglobinaemia / myoglobinuria. (8)
c) Tabulate the difference between Normal saline 0.9%, Ringer’s Lactate and
