WRECK REMOVAL SURVEY REPORT - GUIDANCE NOTES FOR SURVEYORS

We request surveyors to take good notice of below guidelines prior to attending on board.

Important; The surveyor must always settle the survey fees with the vessel’s Owners directly, however,
the Consortium remains in full the main Principal to report and answer to for the surveyor during the
survey.

Complete the survey report with the information available and obtained on board. The survey report should
primarily represent objective evidence, facts and visual observations made on board only. In the reported
deficiencies no reference is to be made to any staff or crewmember on board.

In addition to inspecting the vessel as per guidelines, the surveyor will be required to conduct the following tests:

 CARGO HOLD HATCHES (general cargo vessels only)


Cargo hatches need to be reasonably tight. Pontoons not wasted or heavily corroded.
 HOLDS / CARGO COMPARTMENTS
Bilge system should be operational without back flushing.
 BALLAST TANKS
Surveyor should internally examine a number of selected ballast tanks of various types (i.e. 1 double
bottom tank, forepeak tank, 1 wing tanks etc). All ballast tanks should be structurally in sound condition.
Frames and/or webs should not be wasted, cracked or holed.
 BRIDGE EQUIPMENT
Which items ofthebridge equipment should be tested is left to surveyor‟s discretion, pending on visual
condition.

If a test or inspection could not be carried out, aclear reason for same should be stated. This non-
accomplished inspection must be stated on the deficiency list in the non accomplished item section.

We ask the surveyor to give priority to structural integrety, with regards to the frames, tanktop and deck..

All noted deficiencies must be reported regardless if Owners are carrying out repairs or not at the time
of survey. Reporting must be done by the surveyor directly to the shipowner and made available to The
Consortium upon request. Reports need to be kept available to The Consortium for a minimum of two
(2) years after the survey date.

If in the opinion of the surveyor the vessel is considered to be a bad risk, the full survey report, with colour
photographs and any other supplementary material, copies of certificates, should be forwarded to The
Consortium within the period of maximum 15working days.
Please note that The Consortium requires only ONE copy of the report. The preferred method of receiving the
survey report with enclosures and photographs is by digital version only, if same is not possible we kindly ask to
provide the survey report in hard copy. Please also send a copy of the report to the Owners.
We require at all times photographs from each and every deficiency found and reported, as far as
practicable and reasonable. Moreover, we require various relevant so-called overview photographs in order to
assist us with our own opinion forming.

General guidelines on the quantity of photographs are to be taken as follows;

- Hatch cover, 2 overview photograps of each hold


- Bridge and navigation, 3 overview photographs.
- Hull and Deck structural condition, 3 overview photographs.
- Internal structural condition, 10 overview photographs.
- Machinery spaces, 4 overview photographs.

As such we require a minimum of at least 22 photographs for our review with the survey report.

The condition rating section on page 1 is used to assist to prepare your risk assessment. Considerable
objectivity is required from the surveyor. To ensure a common basis in completing this section, the following
brief guidelines should be followed. Whole numbers with decimal digits can be used.

Page 1 also includes a „summary‟ section. This should be a brief summary of the vessel‟s condition.
The last page of the form should list any deficiencies, clearly stating the nature and extent of these defects. The
Master or other Owners‟ representative must sign the last page of the form, with the list of deficiencies. The
surveyor must explain that the survey observations made may be forwarded to The Consortium who will
subsequently inform the Brokers or Assured as appropriate.

For almost all items within the guidelines a choice must be made from the 4 columns (satisfactory, deficiency,
not inspected or not applicable). In the column „deficiency‟ the number of the corresponding deficiency / non-
accomplished inspection on the last page must be filled in. The other 3 columns must be marked with X or √.

SUMMARY

The Pre – entry Oil Pollution & Wreck Removal survey of MV. “INTAN 18” was conducted on 20 May 2021, whilst
she was berhthed at Sunda Kelapa Port, Jakarta. During the survey, we accompanied/assisted by the Master,
Chieff Officer, Chief Engineer, Deck Officers, Engine Officer.

The vessel was a single hull steel General Cargo with dimension 57.65 x 10.80 x 6.0 (meter). Vessel was powered
by 1 unit Main Engine 5.800 PS x 385 Rpm, Matsui 6M26KGHS. The vessel also equipped with 1 (one) unit
Auxiliary Engine of 100 PS x 1.800 Rpm, Yanmar 6CHL-TN. The Engine Room was located at aft of the vessel. The
vessel was provided with 3 (three) deck i.e 2 (two) Crew deck and Bridge deck, also equipped with Emergency
Generator which were situated at Crew Deck. The vessel has severals tanks such as Ballast Tank (1 P/S, 2 P/S, 3
P/S), Fresh Water Tank (forepeak & afterpeak tanks), and FO Tank (1 C). the vessel was built at Matsui Iron
Works. Co. , Ltd, Japan 1988 and registered in Jakarta, Indonesia.

The vessel equipped with various pumps, Purifier, Compressor, Hydraulic Anchor Windlass at Forward, Hydraulic
winch Cargo Hold and mooring, Oily Water Separator, Bilge Alarm, SOPEP Lockers, Bunker Connection, Sounding
Pipe and Tank Air Vents. The vessel was operated as a liner vessel which serviced on route Bangka Port, Bangka –
Sunda Kelapa Port, Jakarta v.v to load Cargo with speed service about 10 – 11 knots.

The vessel was manned by all Indonesian crews which complied with Minimum Sfe Manning Certificate. The
Master, Officers and Engineers helds valid Certificate of Competency and Endorsment Attesting the Issue of a
Certificate under The Provision of the International Convention on STCW 1978 as amended. The working language
in Bahasa, work as a harmonize team work, diligent, good attitude and competent to operate the vessel.

The list of deficiencies had been provided to the Master and Owner Surveyor and they verbally confirmed to rectify
the deficiencies.

This entire report, together with its summary of defects and / or recommendations for action (if any), has been prepared conditional upon the clear
understanding that in no circumstances whatsoever shall the Insurer be held responsible and/or liable for any inaccuracy in the report, or for any act,
omission or negligence of its staff or its appointed surveyor howsoever arising and that it is provided for the sole reference of the Insurer and without
warranty as to the accuracy of any information or advice supplied therein.

GENERAL COMMENTS ON SURVEY FINDINGS BY SURVEYOR

Briefly remark on the following subjects :


- MV. INTAN 68 serviced as Liner vessel loaded with General cCargo from Sunda
Kelapa Port, Jakarta – Bangka Port, Bangka v.v (Indonesian Water Area)
- The navigation and communication i.e : Radar, GPS, Navtex, and AIS were found
sufficient and satisfactoy condition.
- Ship management System Manual and Procedures had been implemented.
- MV. “INTAN 68” was manned with indonesian crew, The Master and all of Deck/Engine
Officers and crew holding valid certificate STCW 2010 and endorsed on board
particullar vessel.
