R-1-4 – Demonstrate knowledge of social determinants of health and how they

impact individuals, groups and communities.

Describe one significant learning experience in this course related to the CLPNA
competency. Provide an example.

One significant learning experience I gained which relates to the CLPNA

competency was through attending class. During class we identified the determinants of
health and discussed how and why they are important to include when assessing a patient
to understand their situation in a holistic way.

Describe what you learned and how it relates to the competency. Explain why it was
meaningful.

Through attending class I learned that there are 12 different determinants of

health. Some of the determinants such as income and social status have greater affect on
an individual’s health. However, a deficit in any of these areas will reflect negatively on
someone’s overall health. All the determinants of health are extremely interconnected. If
someone has a low income and social status, this will impact all the rest of the
determinants of health too. Determinants of health impact individuals, groups and
communities because they cause the individual or group to become stuck in a certain
place. Making it very hard to break through the cycle that they live in. This knowledge is
important because it shows how to understand the determinants of health and how they
can either help or hinder different groups from achieving optimal health.

competency. Explain what was done to achieve the competency.

Proficiency rating: Good

During my studies and practice in the health care field, I believe I have a good
proficiency rating. To achieve this rating I have studied literature and attended classes.
However I have only spent 4 months overall in the clinical setting so I have not had
extensive hands on time to master this competency.

Describe how you will apply this learning in your current and future nursing
practice.
 In my future practice I will apply this learning by being aware and sensitive to
people in all walks of life that are affected by different determinants of health. I will be
able to provide resources and help for individuals, groups and communities.
