CONTRACTORS REQUIREMENTS
The contractor must observe the following requirement during work:
1 Coordinate work with owner coordinator 2 Comply with Building Fire Safety guidance from Security
3 Follow no smoking rules within the building 4 Surrender valid CPR and obtain access card from
security
5 Keep personals within assigned job areas 6 Do not misuse any of the Fire Fighting Equipment

HOUSE KEEPING REGULATIONS
Good House Keeping and storage practices shall be maintained for all matters and equipment on site without blocking
fire zones and building exits. All debris shall be cleaned by contractor on each day.
UNDERTAKING
 Understand the fire safety requirement and will abide at all time within the work area.
 Will be responsible for the work place if I become aware of danger to myself or others.
 Agree to abide any other fire safety requirement not included above. But find necessary during the period of
work within the premises.
 This form is only a work permit and not a technical or professional release. The contractor is full responsible for
his work.
