“that the IG main line temperature (after IG blower) was high, The IG blower tripped twice and boiler
uptake valve automatically shut. ARter resetting. 1G blower again started and the problem persisted

WHAT WAS THE DIRECT CAUSE Suspected non-return valve on the IG line was
stuck in one position (partially open position) as
the deck IG isolation valve was checked and
found to be open and IG flow was also checked
‘before and afier the deck seal through the cocks
and was found to be sufficient,

shuck in partially open position, due to which the
deck line pressure was not rising and the
temperature of the IG was high and IG blower.

SUMMARY OF THE INCIDENT AND ANY WITNESS STATEMENT IF REQUIRED
During starting cargo operation, it was observed that the IG temperature after the IG blower was high and was near IG tripping temperature.
After 5 mins, IG tripped on high temperature and boiler uptake valve also shut automatically. All the valves on IG line and scrubber line were
rechecked and after resetting the alarms, IG blower was sterted again. But iG blower again tripped due to IG high temperature, To reduce the
temperature of 1G the atmospheric valve was opened 50% and IG was started. It was also confirmed from CCR that all IG valves on deck were
open. Since the atmospheric valve was now open, the IG temperature reduced but the IG deck line pressure was not rising. The atmospheric
valve was operated manially severai times to raise the IG line pressure and aso to ensure that it did not trip on high temperature.
AILER valves double checked on IG line and found all were normal. All valves on IG deckline was also checked and found to be notmal, The inlet
and outlet of the deck seal was checked by openirg the cocks and it was found IG flow and was normal, It was then suspected that the non=
retum valve may be malfunctioning and may be stuck in partially opened position. The non-return valve washammered lightly and it was
observed that the IG temperature ceme down. As soon as the temperature reduced, atmospheric valve was again shut and IG deckline pressure
graduslly increased to normal level.
The non-retum valve has to be opened, inspected and overhauled at the earliest.
