Regina Gurevich is a 47 y/o female who presents to Physical Therapy with signs and symptoms consistent with physician’s diagnosis of (R)Pain: Knee.  
Pa ent presents today with decreased ROM, Strength, Balance, So   ssue mobility and increased Pain, as well as impairments with Posture, Gait.  
Pa ent demonstrates a need for HEP Educa on.  These de cits limit the pa ent’s ability to perform these tasks: Ascending stairs, Descending stairs, 
Squa ng, Walking.  Pa ent will bene t from skilled therapy to allow the pa ent to meet set goals and return to PLOF, in which the above noted 
func onal ac vi es were not impaired.
Primary Complaint: R knee pain. lateral to medial pain. Worst end of the day. Feels like she can't
Nature of Injury: Pt. reports R knee pain started about 2 months ago. Pt. reports she started
walk fast. ge ng back in the gym and running on the TM. She then took a trip to Russia and was having a
Current Level of Func on: Pa ent reports limita ons with Ascending stairs, Descending stairs, lot of pain si ng on the plane. n the  rst day she was in Russia she was walking and felt her
Squa ng, Walking knee "gave out" and then she had to be in a w/c for 2 days. Pt reports she went to urgent care
Prior Level of Func on: Unlimited with all ac vi es where she had xrays and MRI with no signi cant  nding besides OA and DJD. Was in Russia for 2
Pain Scale: weeks and had to wear a brace for the rest of the trip. Reports she has had a li le pain o  and
Diagnos c Tests: has had lidicaine injec ons last Fri (hasn't helped a lot) on in both of her knees over the years for a couple years but not this bad.Pt. is currently wearing
a so  knee brace at work, is nervous about not wearing it when working long shi s for fear of
hur ng it again. Pt. also reports anxiety about  ying again in August and having the pain return.  
AROM PROM Hip/Knee MMT (0-5 scale) Signi cant Findings:

Knee Flexion  126 136* Hip Flexion ( L2) 4* 4+ Gait/Weight Bearing: Decreased WB on the R LE. Pain at
(140°) Knee Extension (L3) 5 5 mid stance - terminal stance on the R

Gluteus Medius 4* 4+ feet and knees close together. B knee valgus L > R

Strength Comment: 3p hamstring on the R- most reproduces some medial knee pain around MCL  
pain with end range  ex and ext of R hamsring Special Tests Comment: Squat: DL- WB more through the L
LE. Pain in anteriomedial knee. Able to go deeper into
squat with hand support but no change in pain level. Step
up (6"); Decreased eccentric control on the R LE, pain with
WB during step up- needs use of counter for support when
stepping up with the R LE
Joint Mobility:  
Tibiofemoral: WNL.    
 
Palpa on Findings:TTP: R medial jt. line, R pes anserine
bursae, R MCL, R gracilis
Balance:  
Single Leg Standing - Le : > 30 seconds; Single Leg Standing
- Right: 16-30 seconds (Loss of Balance)    
Balance Comment: SLS: L >30 sec (grips with toes), R >20
sec (pain down the side of the knee)- demonstrates ability
to maintain nuetral pelvis bilaterally

The pa ent will be advanced safely and appropriately in order for the pa ent to progress towards his/her prior level of func on. Addi onal exercises will be introduced, with progression to a
comprehensive home exercise program upon discharge, to ensure carry over of func onal gains achieved in the clinic. This treatment plan has been reviewed and agreed upon by the pa ent.
Goals:
