Phonics Assessment

for K-3 Readers

www.thisreadingmama.com

Discover the phonics skills your child or

student needs to work on with quick
and simple phonics assessment!

Specific directions are on page two (2)

of this download.

Please Note:
It works best if you wait at least four
weeks in between assessing the student
on the same word list. This keeps the
assessment fresh and more accurate.

This assessment is only a snap-shot of

your learners’ abilities. You will want to
observe learners as they read and write
to get a better picture of what they
know or don’t know.

Administering the
Phonics Assessment
This phonics assessment is to be administered on learner at a time.

*Before assessing, print the rimes (word family parts) from page 4
onto cardstock. Cut out.
*Give the rimes to the learner, one at a time. Learners manipulate the
rimes to make real and silly words by placing the rimes to the right
side of each onset (beginning letter) on p. 4 of this download.
*Learners should read each word they make.
Prompt: “Place the word ending beside each beginning letter to make a
real or silly word. Read each word that you make. Some words will not
be real words. Do not try to make them sound like real words.”
***
For pages 5 and 6 of the assessment, use a blank cover sheet to reveal
only the line of words that the student needs to read, as seen below.
Prompt: “Do your best to read each word you see in this row. I cannot
tell you the word or give you any hints. Just do your best.”
If a child reads all or only misses one in a row,
write the date completed in that phonics feature’s
box.
If a child misses two or more words in a row,
stop the test. This is a phonics skill that needs to
be taught or reviewed.
