To whom it may concern

Nafisakhan Mamamtkasimova is a patient of mine since 2001 and she had some vision therapy.
During our virtual consultation yesterday, she said she has been experiencing several visual
acuity changes over the past month. After my request she went to the optic center and had
a visual acuity test in Phnom penh. She has been a patient of mine since 2001 and has never had
such dramatic visual changes.
Nafisakhan Mamatkasimova’s Rx was:
July 19, 2019
OD: -1.75 OS: -1.75
Nafisakhan Mamatkasimova’s current:
June 12, 2021
OD: - 1.75 OS: - 3.75
I would like to note that Nafisakhan Mamamtkasimova is been prescribed with Neladex 2 drops
a day 3 times and with my prescription she already ordered an eye glasses. Neladex and wearing
an eye glasses for the first time might cause headache and severe dizziness and she already is
complaining from severe headache and dizziness especially when she looks at the screen of the
laptop during an online classes. I am concerned about her v/a changing so rapidly in the past
month and that she expressed the severe eye pain. I believe my patient Nafisakhan
Mamatkasimova should resign from her current work to focus on her recovery.
Thank you.
