Title: COVID-19 Vaccination

Date Prepared: June 12, 2021

Participants: RHU – Staff, BHW’s, and Participants for 1st dose and 2nd dose (A1) Frontline Workers, (A2)
Senior Citizen, (A3) 18-59 years old with co-morbidities.

Schedule: June 14, 2021; from 8 am – 5 pm at Sta. Cruz Gymnasium.

Rationale:

The Department of Health through the Task Group COVID-19 Immunization Program has developed
a comprehensive plan for vaccine deployment and vaccination. The Philippine National Deployment and
Vaccination Plan (NDVP) for COVID-19 Vaccines, seeks to provide the overall operational guidance in the
implementation of the COVID-19 vaccine deployment and vaccination program in the Philippines.

According to Republic Act No. 115251 also known as “COVID 19 Vaccination Program Act”, the
Philippine government adopted an integrated approach to health development, which shall endeavor to make
essential social and health services available to all people.

Objectives:
1. To promote health and prevent community transmission of COVID -19 in the municipality
through regular vaccination programs to all eligible population in the community.
2. To protect the younger generation (18 years old and below) and susceptible population (Senior
Citizens) of the municipality by acquiring herd immunity (70% of the total population is
vaccinated against COVID 19) through on-going and continuous vaccination programs.
3. To emphasize the importance of shared responsibility to all offices, agencies and communities in
preventing spread, and to totally eliminate COVID-19 in the municipality.

This expenses is charge to GAD, Other General Services –


Support Health Services Program

At the end of this activity, we are hoping that the vaccinated individuals will help promote to the community
the importance of vaccines in protecting us and preventing transmission of COVID-19.
