entire period of wake until the interment of the late Patricio. Friends and
relatives’ donations were limited. The client expressed that her family is really
in crisis situation, thus she sought assistance.

circumstances the client has been through, she is very determined to strive
harder for the sake of her family. She wanted to be completely healed, thus she
sought assistance for her continued home medication.

Based on the above – stated findings, the client is considerately


recommended to be granted financial/medical assistance in the amount of
SEVEN THOUSAND PESOS ONLY (₱ 7,000.00) from the LGU Fund.

Zamboanga del Norte Medical Center, unfortunately after five days she passed

grant helped a lot in their education. Melchie added that he couldn’t rely on his
immediate relative since they are also needy.

The client appeared in this agency seeking out external support for her
medical needs. She is having Contusion Periorbital Hematoma at (R) eye

Client narrated that she was intentionally hit by the father of her live – in
partner using a hard object while they were arguing about something. She
didn’t expect that the old man could hurt her that way, she confided. Glaiza is
incapable to purchase her prescription medicines since she has no financial
means.

Presently, the client and her four children are living together with the
parents of her live – in partner. Accordingly, their relationship with each other
is not that harmonious and at peace but Glaiza endured it for the sake of her

she get no cure. Her disease kept her bedridden and resulted to Pneumonia
which was the immediate cause of her death. Catherine is now burdened with
the remaining balance of the funeral charge (payment of coffin and embalming)
incurred from Gamalinda – Hamoy Funeral Homes since the family’s financial
resources were already exhausted.
Catherine, together with her common law husband and their three
children resides in Barangay Guimputlan of this city. The household members
are depending solely on the family head who is a government employee whose
monthly net take home pay is not even enough for all their daily essentials,
monthly bills and school needs of children. Client confided that her live – in
partner has various bank loans.
On behalf of her sister’s family, the client volunteered in seeking out
assistance since the bereaved husband is also facilitating for the late Siony’s
sepulcher. As a sister, she really wanted to extend help in whatever way she
could.

provision of their daily basic necessities including children’s educational needs
since the family head has existing bank loans. Mary Jean volunteered in
seeking external support considering that their immediate relatives are living

have breathing difficulty since she had a pre – existing heart condition. She
was confined at Zamboanga del Norte Medical Center but after four days she

Jinny and the late Leizl had been together for about six years and were
blessed with two kids. They lived in Barangay Sicayab Bucana, Dapitan City.
The family’s source of living is obtained from Jinny’s fishing activity in which

unexpectedly he collapsed at their home. He was rushed to the hospital but he

farming is their source of living. However, when the household head passed
away, the client finds it difficult to till the farmland all by herself considering
she’s already old and weak. Together with her two grandchildren, they are just
depending on the support in terms of basic material goods coming from her
married children and of her meager social pension as Indigent Senior Citizen.
At this moment, Pacita is bothered on how to defray balance of the funeral
services rendered to her late husband including the coffin which was indebted
from Gamalinda – Hamoy Funeral Homes. With this, client emerged in the
agency appealing of external aid.

It is therefore recommended that the needy client be granted burial


assistance from the LGU, Dapitan City to cope with her present financial crisis.

The client is in dire need of assistance as he finds difficulty to provide for


his medication expenses. Accordingly, he was deliberately mauled by his own
relatives while they were having some misunderstanding during farm works.
He got Laceration 3.5 cm below chin, I cm ® eye, 0.5 and lower lip 1cm ®
eyebrow as per medical certificate attached. Gavino’s attending physician
prescribed that he must undergo for a CT scan to further assess and evaluate if
there is a brain injury since he complained of experiencing severe headache
and dizziness. Unfortunately, the client is financially incapacitated. With this,
he came to office seeking for external support.

41 years old Gavino M. Todara is unmarried; however he has a common


law wife named Jovelyn G. Rendon. The couple has been living together for
more than a year already but not yet blessed to have a child. They reside in
Barangay Sinonoc, Dapitan City. The client does farm labors and sometimes
engages in any odd jobs while Jovelyn is a mere laundress. Their aggregate
income is not even adequate in the provision of their basic material goods. He
couldn’t rely on his immediate relatives considering they are also in need.

From the information gathered above, it is therefore recommended that


client be favorably granted financial assistance from the LGU, Dapitan City.

attending physician. But due to some personal reasons she failed to
continuously take her medicines. According to Manelyn, her sister was brought
by her live-in partner in Katipunan, Z.N. while she’s studying BS IT. The couple
had a child who is now two years old. Sadly when she became weak and her
condition seemed to worsen, her partner had no time to take tender care of her
since he’s also busy with his work being an electrician. The client decided to
take Marchie back in Dapitan City and took the responsibility of being an “Ate”
to her. Marchie was hospitalized but not fortunate enough to regain back her
good health and eventually died.
Manelyn is married to Adam Mark Jatico and they have only one child
named Dency Dave Jatico who is now in Grade IV. The client’s husband is the
sole provider of the family and his meager monthly income as a job order
employee in the city is sufficient only to sustain their daily basic material goods
and son’s school needs as well. As the eldest among five siblings, the client
personally came in the agency asking for assistance to defray the outstanding
balance of the coffin and funeral services rendered to her late sister incurred
from Gamalinda- Hamoy Funeral Homes since limited support from immediate
relatives was exhausted.
It is therefore recommended that client be granted burial assistance from
the city local government.

Family of the client is a recipient of Pantawid Pamilyang Pilipino Program


considering it is one of the less fortunate families in Barangay Dampalan,
Dapitan City. The cash grant that the family regularly receives is a lot of help to
the school needs of Grade 8 and Grade 6 children. Nevertheless, the daily basic
commodities of each household member are barely catered since the primary
source of their living is obtained only from the minimal income of the client

been ill for quite a while coming in and out from the hospital. Just this recent
he was confined at Corazon Aquino Hospital; unfortunately he never recovered
from his illness and died last April 10, this year. Accordingly, the family’s
financial resources were completely used up the reason why the client needs to
seek for another means so the funeral liability which is the payment of coffin
and embalming of her late brother be settled.

The family’s source of living is acquired from fishing/fish vending in


which the irregular and unstable income is sufficient only for their day to day
vital needs. As recognized to be among of the indigent families in Barangay
Napo, Dapitan City, Etoquilla family was chosen to be a beneficiary of 4P’s. The
cash grant is well utilized for the educational expenses of two in school
children and it helped a lot. Support extended by immediate relatives was so

Ril who passed away last July 3 of this year at the age of 29.As per death
certificate attached herewith, he died due to Typhoid Fever with Typhoid
Psychosis. Before he died, he used to work at a private company in Metro
Manila. But just last June, Jay was inflicted with an on and off fever
accompanied with colds and cough and carried it for about a month. He has
not paid much attention on it and never took the chance of consulting a doctor
instead; he medicated himself with herbals and other generic drugs. However,
he gained no cure. He went home and admitted at Dr. Jose Rizal Memorial
Hospital but then again he never recovered from the grueling illness and it
progressed to delirium. According to Arnita, her son became so upset and
unknowing so she decided to let him discharge from the hospital and sadly,
two days after he died.
Arnita is now facing emotional and financial hardship, she couldn’t
afford to defray the outstanding balance of the cost of coffin and 10 days
embalming of her son the late Jay I. Ril, and thus she came to office for
assistance.
Ril Family is known to be among of the less privileged families in
Barangay Burgos, Dapitan City wherein mostly of the community people rely
on farming as their source for a living. The client is a plain housewife while her
husband is a farmer. Living with them is the family of her unwed daughter
whose live-in partner is a driver. The family’s aggregate monthly income is
barely adequate in sustaining the vital needs of each household member.
Immediate relatives’ support during the wake and burial of the late Jay was
very meager.

Currently, the client is an active public servant being a Barangay


Councilor of Barangay Oyan, this city. His nominal monthly honorarium is
sufficient only to sustain his daily vital needs and of his daughter’s. From the
time his common law wife died, he became a single parent to his grade two
daughter Joylen Mayfer L. Acoymo. He admitted that it’s quite hard raising his
child all by himself and taking care of his aged mother as well. However,

An elderly client, Ladisla E. Cadorna is seeking out external aid for the
medical needs of her seventeen years old grandson, Genrey E. Cadorna. The
latter was a victim of a vehicular accident that caused him Multiple Fracture
of Tibia Fibula (L) with External Fixture (per medical certificate attached).
Accordingly, he underwent a certain surgical procedure and its costly package
(including the doctor’s fee) was only compromised by the client with a promise
to settle it the soonest possible time. There were also medicines prescribed by
the attending physician that the family failed to purchase. What happened to
Genrey adds up the economic hardship of the Cadorna’s. With these concerns,
client appeared to office for assistance.

Eighty years old Ladisla is a resident of Barangay Talisay, this city. She

the child was only two years old when the mother (client’s daughter) left for
Manila and opted to live there with her new family. Rarely, the client receives
monetary support from Genrey’s mother which hardly caters the child’s needs.

Presently, the client and her grandson are depending on the meager
support of immediate relatives. Ladisla is just so thankful that she is quarterly
receiving Social Pension of ₱1,500.00 allocated by the government for the
identified indigent Senior Citizens. Somehow, it helps a lot for their vital needs.

Based on the above-gathered data, it is therefore recommended that the


needy client be granted medical/financial assistance from the LGU Funds of
Dapitan City.

Forty-one years old client Josue R. Dangcalan mourned over the demise
of his beloved elder sister, Jelah R. Dangcalan who died of Breast Carcinoma
(L) with Hepatic and Bone Metastasis. Said sister has been suffering from
such abominable illness since March of 2017. In fact, she had undergone an
operation but refused to have chemotherapy and chose to take alternative
medication instead. She was not fortunate enough to surpass and recover from
her cancer and later on she passed away, client narrated.

Dangcalan Family hails from Barangay Potungan, Dapitan City. The


client is into odd jobs while his wife is a government employee whose salary
was being loaned since she is the breadwinner of the family. Household
members’ daily basic commodities are barely sufficed.

Presently, the client is burdened with the outstanding balance of the


funeral services rendered to the late Jelah incurred from Gamalinda- Hamoy
Funeral Homes. Immediate relatives’ help was meagerly extended. In this
reason, client came to office for assistance.

Based on the above-gathered data, it is therefore recommended that

The client is in need of assistance for payment of coffin and 9 days


embalming of his maternal brother, the late Marino Caermare Balladares who
died on August 11, 2018. Per client’s statement, his brother met a vehicular
accident last August 10 along the national highway of Barangay Sulangon, this
city. He sustained Severe Traumatic Brain Injury and didn’t make it to survive.
He passed away the following day due to Cardiac Respiratory Arrest. The
entire family and kin mourned his untimely death.

George and his family are living at his mother’s house located in
Barangay Sulangon, Dapitan City along with his other siblings. Both George
and his younger brother are construction workers. Their mother is a laundress.
Their aggregate income is sufficient only in the provision of the family’s daily
basic material goods. The client shared that he needs to settle the unpaid
balance incurred from Gamalinda- Hamoy Funeral Homes, thus he appeared in
this agency appealing for external support.

From the data gathered above, it is therefore recommended that a burial


assistance from the city government be granted to the needy client.

Infarction. She had a pre-existing Diabetes Mellitus Type II prior to her death
but wasn’t properly given medical attention out of financial inadequacy, client
said. Arsenia is now worried on how to defray the outstanding balance in
Gamalinda- Hamoy Funeral Homes which is the payment of 9 days embalming
and ordinary coffin of her late mother.

Arsenia A. Elumbaring is a resident of Barangay Sulangon, Dapitan City.


She’s a spinster who is now living alone in their house. For her living and
survival, she relies on her quarterly share from the communal coconut farm
profit which is sufficient only for her vital needs. Immediate relatives cannot be
depended since they are also in need. The client finds difficulty to defray the
funeral liability, hence she sought assistance.

It is therefore recommended that the needy client be possibly granted


burial assistance from the city local government of Dapitan City.

CASE SUMMARY

Last August 4, 2018, the client’s beloved better half, the late Bienvinido
D. Javierto passed away at the age of 67. Prior to his death, the husband
complained of having a dizzy feeling and abdominal disturbance, client stated.
He just ignored it with a thought that it has nothing serious to worry about.
Yet, on midnight of August 4, he allegedly got a heart attack after he
confronted the drunk group of noisy men at their neighbor’s store. He then
rushed to the hospital but never made it to recover and eventually died of
Acute Myocardial Infarction. His demise caused so much pain to the whole
family.
The Javierto Family has a small carenderia in Barangay San Vicente of
this city where they reside. It is the main source of the household members’
day to day living and survival which income is barely enough to sustain their
vital needs. Arlen is a mother of 7 children of whom four of them are in school.
They are also recipients of 4Ps. Her eldest helps her in the carenderia. The
youngest child of the client has a hydrocephalus condition that needs special
attention and whose maintenance medicines are expensive. Despite of the
unpleasant circumstance that Arlen’s family has been through, the client
remained strong for the sake of her children.
The client confided that she is presently burdened with the remaining
balance of the funeral package (coffin & embalming) rendered to her deceased
husband. She needs to settle the liability in Gamalinda- Hamoy funeral Homes.
However, she is financially exhausted. In this reason, she appeared to office for
assistance.
From the above stated findings, the client is hereby recommended to
avail of a burial assistance from the city government of Dapitan.

Seizure Disorder (Epilepsy). He had maintenance drugs but he cut taking them
two months prior to his demise.

Family of the client is a recipient of 4Ps. Two of in school children


benefitted the monthly cash grant which is of great help to their educational
expenses. Ma. Mila is a plain housewife who manages the household and
attending to the daily undertakings and routines of each member particularly
the younger children. Her husband is a mason whose irregular income hardly
caters the family’s immediate needs.

Presently, the client is in need of outside support as she finds difficulty


in paying the funeral liability incurred during the death of her son, Kelly Clem.
The funeral parlor she had indebted with coffin and embalming is already
asking for full payment. With this, she came to office for assistance.

It is therefore recommended that Sagapsapan Family be given burial


assistance from the City local government.
