III. SOURCES OF INFORMATION

The sources of information of the case is the client itself and their family, through
interviews and direct observation of the Field Instruction student who made her home
visitation to the client using open-ended questions.

IV. PRESENTING PROBLEM


His presented problem is his illness on his kidney. He has been experiencing
this for already about 3 years. He had only sought help from the Rural Health Unit for
his medicines, and at times he uses herbal techniques when he cannot afford to buy
for his medicines. Time after time he has difficulty in urinating.

V. BACKGROUND INFORMATION

A. Client

Wenefredo Pamaos is a 61 years old man, currently he is residing at


Alcober Subdivision, Poblacion District V, Burauen, Leyte. He was born on June
15, 1960 at Poblacion District 1, Burauen, Leyte. He is a High School graduate and
finished High School education at Burauen National High School.
He has a low productive work and earns just enough to feed himself and his
children. He worked as a factory worker before and currently selling “balot” and
earns more or less ₱200 a day. Mr. Pamaos is self-employed but frequently
experience problems in urinating that affect his work. He has problem with his
kidney for 3 years from now. He needs medical assistance for the examination and
treatment of his kidney problem for his money is just enough to feed his family. He
is the only one who supports his children in studying, and the only one who help
him in earning is his eldest son who earns a few pesos occasionally, working as a
laborer.
Mr. Pamaos is a smiling person, he has respect to others, a responsible
father, also he is an accommodating man and he has an optimistic attitude towards
life though he and his family faces a lot of problems. Even though, he and his wife
have been separated, he is trying to have good terms with her for their children.
B. Current Family Functioning

They are 4 in the family, including him, and their 3 children. He and his wife
have been separated for about 2 years. His wife is living currently at Dagami, Leyte
living with a new family. Occasionally, she visits their children and her mother in
Burauen, Leyte.
His son is working as a laborer as classes are held online, and his younger
children stays at home and oftentimes go to nearby relatives during the day to play
with their cousins. The eldest son is currently a 2nd year college student at EVSU
Burauen taking up Bachelor of Science in Secondary Education. The second
child/eldest daughter is a Grade 11 student and the youngest daughter is a Grade
7 student.

VI. ASSESSMENT

Mr. Pamaos has only used limited internal resources for solving his problem, he
has not used resources available to him. If he has sought help, his problem with his
kidney would already be solved. He tried to solve his kidney problem by using herbal
techniques for his earnings are only enough for food expenses and would not suffice
if he will buy for his medicines.
At this time of request for help, Mr. Pamaos’ family functioning is at low level of
socio-economic adequacy, and his problem really needs immediate and outside help.
Given the client’s situation, though some of his physiological needs like food, clothing,
and shelter are provided by him as this was his first priority, his health has been
compromised for the reason that he is not able provide for his medical needs. Though
he asks assistance from the local government through RHU, still it is not fully
compensated, and that is the reason why he is suffering from severe kidney disease
at present.

VII. TREATMENT PLAN

Goal: To help the client restore his social functioning by referring him for medical
examination and treatment of the client’s kidney problem.
Specific Objectives:
1. To refer Mr. Pamaos in the Internal Medicine Doctor/Nephrologist at Eastern
Visayas Regional Medical Center.
2. To refer Mr. Pamaos to the Department of Social Welfare and Development for
livelihood assistance to ensure better employment.
3. To undergo family counseling to improve family relationship.

VIII. RECOMMENDATION
It is recommended that Mr. Pamaos be referred for the treatment of his kidney disease
immediately. Also, it is needed that he receives livelihood assistance for better
employment to uplift the economic status of their family.
Furthermore, he and his family undergo in family counseling to improve the family
relationship.
