II. FAMILY COMPOSITION

Jotham R. Borda : 12 years old, single, grade 6, son, student
Marie Jo B. Locluran : 27 years old, married, daughter

III. PROBLEM PRESENTED

Client prompted to seek possible financial assistance because her husband Jose Borda was
suffering From Humeral Shaft Fracture (L) closed complete. Now, a big amount was incurred thus
the request for financial assistance.

IV. FINDINGS
Marilou Rafol Borda is 52 years old. Her husband Jose Borda who was a stroke victim since
2015 who is now living with his live inn partner for so many years is now experiencing humeral
shaft fracture (L) closed complete which hinders the mobility considering the fact that accident
cannot be avoided . Subject for assistance is suffering from a frozen shoulder for almost three years
due to secondary to stroke. Since he was victimized by a stroke his physical mobility was greatly
affected and all his physical activity was all paralyzed. Jose the client has difficulty in walking and a
sudden turn caused him to suffer a bad fall where her left shoulder was now then problem. The
patient has to suffer pains where tramadol was then prescribed by the attending physician. The
situation of the client who was victimized by a stroke, his right to mobility at present was no
deprived and the physical situation was aggravated. His shoulder was affected thus his physical
activity was now a big burden. Truly, the patient at present needs preferential medical attention
considering his present ill health condition.
The victim therefore who is already in his mid-fifties chances of healing such any kind of
fracture will take long due to age. Getting old made our bones fragile so necessary precaution and
preventive measures. Curative medical intervention is so exp0ensive thus preventive medical
intervention is practical enough on the part of the victim. To avail medical intervention to private
hospital calls for augmentation of the financial resources available in the community like the
 provision of medical assistance. Economic stability of the immediate family suffered financial crisis
as subject for assistance really cannot afford. Now clients live inn partner is now his care giver while
the real wife has to source fund purposely for the recovery of the victim.

VI. RECOMMENDATION
Based from the above findings and justification client is very much eligible to financial
assistance.
