III. PROBLEM PRESENTED

The client is belong to the survival level based o the result of SWDI (Social
Welfare Development Indicator) who is seeking for livelihood assistance to
support the needs and education of the family.

IV. BACKGROUND INFORMATION

Ms. Consuelo P. Sebastian, 43 years old, born on February 25, 1974, his
husband is Mr. Freddie Sebastian, 43 years old. They have 5 children, 2 girls
and 3 boys. Katherine Sebastian the eldest among the 5 siblings. She was
19 years old, born on February 21,1998 at Malawaan Rizal Occidental
Mindoro. The second child is Kim Charlon Sebastian, 17 years old, born on
March 25, 2000. Followed by John Clark Sebastian, 15 years old he was born
on August 04,2002. The fourth child is Jerick Sebastian, 13 years old he was
born on April 28, 2004 and the youngest is Caren Sebastian, 10 years old.
She was born on May 17,2007 at presently in Grade 6 at Malawaan
elementary school.

V. CURRENT FUNCTIONING OF THE FAMILY

Debora Mendoza has three children. Her first child is Jode Andre,
presently studying at Malawaan elementary school, he was in grade V; during
weekends he was the one who help her mother selling their products. As the
first child he can identify and understand their present situation, and his role
as the eldest. He acts as mediator when her sisters has an argument.

Joyce anne 10 years old, the second child and studying grade IV at
Malawaan elementary school. As the second daughter she took all the
responsibilities of household chores if was her mother was not around. The
reason why she and her youngest sister always quarreled.
Jhamine was 6 years old and the youngest, she was always the reason
why their family was happy every day. She is lovable, sweet but sometimes
she become importunate maybe because she was the youngest and the
center of affection of the family.

VI. ASSESMENT

The family was not among to the beneficiaries of Pantawid Pamilya


Pilipino program. Based on the findings and observation that I gathered the
client really needs a livelihood assistance to augment to able to support her
existing project which is selling halo halo and others cooked foods.

She did not asked help from her family since they were living in
Magsaysay and very far from them.

The client is eligible from livelihood assistance to augment their present


small business and meet the needs of the family.

VII. EVALUATION/RECOMENDATION

Based from the gathered information about the client Mrs. Debora P.
Mendoza she want toachieve the following goals;

a. Refer the family to avail the livelihood assistance to support the needs of
the family through the intervention of government agencies.
b. Develop her skills to managing her small business.
