II. FAMILY COMPOSITION

Dolia Rodelas : 81 years old, married, mother, elementary level.

III. PROBLEM PRESENTED

Client seek possible medical assistance for he is suffering from PTB/ Pneumonia was advised for
chest x-ray but financially hard- up to finance his transportation and other medical expenses going to
hospital of choice at Roxas City, hence this request.
IV. FINDINGS
Arturo Romano is 46 years old, a bonafide resident of Barangay Poblacion, San Fernando,
Romblon, single and living with her mother as the breadwinner of the family earning very minimal
income through farming which is very insufficient to support family needs. To further augment
family income client mother accept sewing like repairing dress, and rearing domestic animals like
pigs and chickens.
Client ailment needs long medication and maintenance medicines is given free at RHU but
limited only. Client family can’t afford to support other medical needs thus, seeking for medical
assistance for client immediate recovery.
V. ACTION TAKEN

1. Office interview of biological mother;

2. Made Social Case Study Report;
3. Extended Medical Assistance (AICS);
VI. RECOMMENDATION
This is the case of client suffering from PTB/ Pneumonia, needs further treatment, family
income is not fixed therefore, the worker recommends to avail the medical assistance.
