III. PROBLEM PRESENTED:

Mrs. Ana personally came to this office requesting a Social Case Study Report for her son’s
medical treatment.

IV. FINDINGS:

Client disclosed that subject is in and out in the hospital due to his condition. Accordingly,
subject has been suffering from his illness since 2015. Before his confinement, Ana averred that
patient could hardly get up and couldn’t walk. His knee was swelling and so Ana decided to bring him
in the hospital. On August 24, 2020 patient was admitted at ___________ Hospital of _________. He
was treated there for more than three (3) weeks. During subject’s confinement attending physician
diagnosed him with HEMOPHILIA A MODERATE WITH INHIBITOR SEPTICEMIA. Ana disclosed that due
to her son’s condition attending physician advised patient to undergo operation. On August 30, 2020,
patient then has successfully undergone IJ CATHETER RIGHT and BELOW KNEE AMPUTATION RIGHT
operation. But after the operation, his neck became swollen and there were episodes of difficulty in
breathing that required another operation. Thus on August 31, 2020, patient undergone another
surgery (TRACHEOSTOMY). Ana disclosed that after long weeks of confinement, family opted for
home medication and was untimely discharged from the hospital on October 16, 2020 due to financial
reason.

Ana confessed that she was separated from her husband when her son was only 3 years old.
She shouldered all the responsibility of raising Ramon alone without the support from his father. Year
2011 when Ana met her partner Perry. They decided to live together. At present they are living in
small house made of light materials. Client revealed that she could no longer afford to finance the
medical expenses of her son considering her little income. She had incurred many debts because of
her son’s previous treatments and confinement. Subject was also undergoing follow-up check-up
every month for evaluation and monitoring of his condition until present.

Due to subject’s delicate condition his grandmother is the one caring for him. Ana averred that
her parents are living with the family to help her take care of her son. This is also a big help on her
part because she has no finances to pay the caregiver/house helper. Client further revealed that
because of financial incapacity, relatives could not even extend help for subject’s needs. And so, Ana
sought assistance from the government.

V. EVALUATION AND RECOMMENDATION:

This is a case of 15 years old boy who undergone operation and presently undergoing
medical treatment every two weeks. Patient’s mother expressed difficulty in terms of finances
because family source of income is limited. Despite all these, still she has positive outlook as she
claimed all these are just part of life’s trial.

As per interview and information gathered from patient’s mother, subject Ramon G. Nando,
was found in need of assistance. The worker would like to recommend client/subject for possible
help/assistance, which your institution/agency could best extend.
