II. FAMILY COMPOSITION

III. PROBLEM PRESENTED


Client husband prompted to seek possible social case study, for him to ask financial assistance
from the Philippine Charity Sweepstakes Office (PCSO).

IV. FINDINGS
will suffer economics crisis to be afflicted with kidney chronic diseases stage 5, is
reallyAlfredo M. Ravalo , 61 years old, married and presently residing at Poblacion San Fernando,
Romblon . As disclosed by the client during the confinement of his brother in law where the initial
diagnosis was chronic kidney disease thereof advice to undergo twice a week dialysis. Twice a week
dialysis calls a bigger budget but the immediate family by all means adhere to doctors’ advice which
made the whole immediate family socially, psychologically and economically disadvantaged while
subject for assistance still undergoing medication. The family of the client however is the sole
financer; hence his brother in law who was so ill is the only brother of his wife. So, as an extended
family and to prolong the death of Cecilio Manalon the family’s by all means borrowed money from
lending institution and cooperatives just to sustain the medical treatment of the patient.
Furthermore, subject for assistance was a stroke victim for almost seven (7) years now. The
care giver of for the patient for seven (7) years was his wife but cannot afford to shoulder the
expenses of his ill husband because they belong to food threshold family thus they can afford only to
finance basic need. So, the family of the client has really no choice but to help Cecilio Manalon, a
victim of acute stroke and chronic kidney disease. Since the confinement of the patient from March
06, 2018 until the last breath of the patient, last April 17, 2018, the family of Mr. Alfredo M. Ravalo
suffered financial constraint. Subject for assistance therefore, was put to Intensive Care Unit of
Ospital ng Muntinlupa from April 4, 2018 to April 17, 2018 where the prescribed medicines daily
amounting to Ten Thousand pesos made the family of Alfredo M. Ravalo financially disadvantaged.
Despite financial help coming from their loving and benevolent relatives. In fact the hospital bill
recorded at Intensive Care Unit amounting to One hundred Thirty Three Thousand Pesos (P133,
000.00) was at stake. Although Phil health, PCSO augmented in paying the hospital bill still the
balance unpaid hospital bill worth Fifty Seven Thousand Pesos (P57, 000.00) was a problem.
However, family’s financial resource was not yet identified as to where to extract.
Both external and internal resources of the immediate family were already maximized but still
the amount accumulated cannot really warrant. Now the big burden is the burial expenses incurred
worth Fifty Four Thousand Pesos (P54, 000.00) thus, opted ton seek assistance from the Municipal
Social Welfare and Development Office the champion inn alleviating the plight of the socially and
economically disadvantaged family who are in crisis situation. No possible source cannot be
identified at this point in time.
V. ACTION TAKEN
1. Conducted Personal Interview;
2. Made Social Case Study Report;
3. Provided Counseling
VI. RECOMMENDATION
Based from the above findings and justification, this is a case wherein possible family’s financial
resources was maximized just to prolong the death of Cecilio Manalon , the deceased but no choice
last April 17, 2018 at Ospital ng Muntinlupa , the patient died due to acute respiratory failure
secondary to aspiration , pneumonia and acute hemorrhage stroke with intru ventricular extension
end stage renal disease ( ESRD).
In view thereof, the undersigned respectfully recommends possible burial assistance to the
bereaved family of Cecilio Manalon. Any possible assistance is highly appreciated
