II. Sources of information and circumstances of referral:

A letter from the Inter – Country Adoption Board (ICAB), dated May 25, 2015 was received by Miss
Meredy Mar on June 10, 2015 referring Princess Dianne Estores Pantonial for case management and
eligibility assessment.
III. Background Information :
A. The Child
Description of the Child upon Entrustment

The Child Princess Dianne Estores Pantonial was directly entrusted by his biological parents Darryl Pantonial
and Rae Ann Estores last sometime August 2012 when Princess Dianne is 2 months old. She is wearing white
baby dress with mittens and white sacks and white baby shoes with some pieces of second hand baby dress. She
weight 6.0 kilos and looks pale and malnourished as described by her grandparents.

B. Medical and Developmental History of the child

A baby girl Princess Dianne was weighing 5.7 lbs. upon delivery with spontaneous normal delivery thru the aid
of a midwife Mrs. Elena V. Encavo. She was breast feed for two months and eventually a substitute infant
formula was given since two months old and when she reaches 1 and1/2 year a fuel creamed Nido formula
became a substitute. In order to keep her nourished a lactum formula became an alternate however signs of
diarrhea manifested, thus her attending doctor advised her grandmother Elca to stop feeding in order to evade
future stomach complaints like diarrhea.

Princess Dianne started to eat solid food when she was four (4) months old. To mentioned are mashed banana
fruits, mashed boiled, such as squash, carrots and potatoes as well. She was nurtured by her grandma and
grandpa who are both professionals specially her gran mom is a teacher , the love and affection that a child
needs to learn and recite nursery rhymes and most importantly introducing how to say a prayer.

Social manifestation at three years old ,Princess Dianne being friendly because she gained many friends not
only of her age but those older than her . She can easily adopt with her playmates but her constant playmate is
her one year old cousin Rayden where she used to her best buddy.

At three years old, Princess Dianne can identify different types of animals and shapes. She can solve simple
puzzles like matching objects. She knows how to play educational games on electronic gadgets such as tablets,
laptop and smart phones. As an observant child, she can open and use Skype thus; sometimes she was able to
call her Mommy Anna and Daddy Louie in New Zealand by chance without any knowledge on her
grandparents. She can memorize and can sing nursery rhymes like twinkle twinkle little stars, 10 little Indians,
Rocking the boat. She simply doesn’t get tired viewing her two favorite movies tinker bell and Frozen and
performs what the lead character doing. As she is introduce at early age to recite rhymes and prayer, at three
years old she can recite prayers like our father and Hail Mary. She can even right her full name, her names and
even her adoptive parents. As an observant and a smart child she can recite already the English alphabet
correctly and can even count 1- 10 and up to 20 with a help.

She can dance well using her legs and arms simultaneously. She can ride in a three- wheeled bike and electronic
toy car without a help of an adult. She can run well without hurting herself. She can drink alone in a glass or a
cup and eat by herself using spoon and fork. She walks upstairs and downstairs freely using alternate feet.
Besides she can talk well with whom she talk with but sometimes she still find difficulty in pronouncing words
that are hard to pronounce.

She sometimes manifested tantrums but can easily be pacified. Aside from being friendly she loves to bring the
center of attention. As a sweet child, she likes to be cuddled, hugged and kissed by her mom and dad,
grandparents, aunts, uncle but most especially by her small cousins.
Princess Dianne at 6 years old thru her psychological report her intellectual functioning shows to have high
average intellectual skills and abilities , Her verbal comprehension composite score of 109 falls on average level
while her perceptual reasoning composite score of 120 falls on the superior level.

On the verbal comprehension, which is classified as average, Princess is shown to have adequate knowledge of
the meaning of words. Likewise she has the capacity to understand underlying of objects and ideas, which
suggest that she can lean new materials quickly.

On the Perceptual Reasoning Level which is classified as Superior, Princess Dianne is shown to have a capacity
to quickly coordinate her eye hand movements to synthesize ideas into a meaningful whole.

Scrutiny of Princess Dianne T scores and age equivalent coordination’s has an age equivalents of 7 years and 2
months. Her word knowledge skill has an age equivalent of 6 years while her capacity to grasp object idea
relationship has an age equivalent of 9 years and 6 months. Additionally her ability to understand ideas in
abstract form have and Age Equivalent of 6 years.

Overall, IQ result reveals that princess is well attuned to task that call for visual motor coordination pattern
analysis and sequential thinking. She can quickly process information that can be conceptualized in figures,
objects and patterns. Given her young age she can still improve and enhance her skill and abilities through
motivation, good study habits, self-discipline and an open and enthusiastic attitude to new learning.
